ObjectivesThis study was to investigate the influencing factors of coronary atherosclerotic heart disease in women,to understand the relationship between coronary heart disease and influencing factors,to analyze the influencing factors of women with coronary heart disease,and to provide a scientific basis for the prevention of coronary heart disease in women.Promote cardiovascular health in women’s groups.MethodsThis study selected patients admitted to the Department of Cardiology of Shandong Provincial Hospital as the respondents.The survey period was from December 15,2017 to December 15,2018.Collect basic information about patients through questionnaires.SPSS 17.0 software for data analysis.The relationship between coronary heart disease and influencing factors was studied by t-test,chi-square test,analysis of variance,rank sum test,bivariate correlation analysis and logistic regression.P<0.05 was considered statistically significant.ResultsIn this study,a total of 1232 subjects were collected,including 410 female patients with coronary heart disease,410 patients in the control group,and 412 male patients with coronary heart disease.The age and education level of the subjects were statistically significant between the case group and the control group.The prevalence of BMI≥24,hypertension,diabetes and dyslipidemia in women with coronary heart disease is higher than that in men,and the difference was statistically significant.There were 290 menopausal women with coronary heart disease and 410 women in the control group,there were significant differences between the menopausal group and the control group in age,marital status,and education level.1.BMI≥24,sleep time(>8 hours/day),menopause,menarche age≤14 years old,number of births>3,passive smoking,hypertension,diabetes,dyslipidemia,family history of coronary heart disease,TG,TC,LDL-C,HCY,GA,CRP are independent risk factors for coronary heart disease in women.Physical activity time(>2 hours/week),sleep time(6~8 hours/day),HDL-C are protective factors for women with coronary heart disease.Multi-factor analysis shows that BMI≥24,sleep time(>8 hours/day),menopause,menarche age≤14 years old,number of births>3,passive smoking,hypertension,diabetes,dyslipidemia,family history of coronary heart disease are risk factors for coronary heart disease in women,the OR values were 3.25(95%CI:1.35-8.72),3.32(95%CI:1.15~3.12),3.15(95%CI:1.23~2.77),6.84(95%CI:4.22~12.28)and 3.36(95%CI:2.13~5.87),1.57(95%CI:1.12~2.43),3.41(95%CI:2.30~5.61),1.71(95%CI:1.16~2.79),2.65(95%CI:1,11~2.58),11.74(95%CI:5.73~26.66).TC,TG LDL-C,HCY,GA and CRP were risk factors for coronary heart disease in women.The OR values were 1.87(95%CI:1.33~2.92),3.57(95%CI:2.53~5.59)and 3.53(95%CI:2.62~5.28),1.67(95%CI:1.14~2.72),1.77(95%CI:1.25~2.79)and 1.66(95%CI:1.10~2.76).Physical activity time(>2 hours/week)(OR=0.22,95%CI:0.06~0.86),sleep time(6~8 hours/day)(OR=0.49,95%CI:0.28~0.96),high-density lipoprotein cholesterol(OR=0.39,95%CI:0.29~0.59)are protective factors for coronary heart disease in women.2.HS-TnT,CKMB-M,MYO,NT-ProBNP are positively correlated with the occurrence of coronary heart disease in women(P<0.05,P<0.05,P<0.05,P<0.01).3.The higher the age,the higher the Gensini score of women with coronary heart disease,and the Gensini score increased in female coronary heart disease patients with family history of coronary heart disease,passive smoking,hypertension,diabetes,menopause.BMI>24,low HDL-C,and high LDL-C.4.The risk of coronary heart disease in menopausal women with BMI>24 was 3.32 times(95%CI:1.39 to 8.77)for subjects with BMI between 18.5 and 23.9.The risk of coronary heart disease in women with sleep time>8 hours was 3.16 times(95%CI:1.31 to 8.42)for subjects with sleep time<6 hours.Compared with women with a menarche age of 15 to 18 years,the risk of coronary heart disease was 1.70 times(95%CI:1.15 to 2.79).The number of births>3,passive smoking,hypertension,diabetes,LDL-C are risk factors for coronary heart disease in postmenopausal women.The OR values are 3.27(95%CI:2.01 to 5.88),1.81(95%CI:1.25 to 2.71),2.11(95%CI:1.20 to 3.21),2.41(95%CI:1.51 to 3.84),1.72(95%CI:1.14 to 2.87).High-density lipoprotein cholesterol(OR=0.35,95%CI:0.24 to 0.55),sleep time(6-8 hours/day)(OR=0.39,95%CI:0.25-0.69)are protective factors for menopausal women with coronary heart disease.5.Before menopause,the proportion of coronary artery single-vessel disease in female patients with coronary heart disease is higher,while the proportion of double-vessel and multi-vessel disease is lower,and most of the lesions are located in the anterior descending branch.After menopause,the range of coronary lesions in women with coronary heart disease increases,and the degree of lesions increases.Conclusion1.BMI>24,sleep time(>8 hours/day),menopause,menarche age<14 years old,number of births>3,passive smoking,hypertension,diabetes,dyslipidemia,family history of coronary heart disease,TG,TC,LDL-C,HCY,GA and CRP are independent risk factors for coronary heart disease in women.Physical activity time(>2 hours/week),sleep time(6-8 hours/day),HDL-C are protective factors for women with coronary heart disease.2.HS-TnT,CKMB-M,MYO,NT-ProBNP were positively correlated with the occurrence of coronary heart disease in women.3.Gensini scores in female patients with high age,family history of coronary heart disease,passive smoking,hypertension,diabetes,menopause,BMI>24,low HDL-C,and high LDL-C.4.BMI≥24,sleep time(>8 hours/day),menarche age≤14 years old,number of births>3,passive smoking,hypertension,diabetes,LDL-C are independent risk factors for coronary heart disease in postmenopausal women.HDL-C and sleep time(6~8 hours/day)are protective factors for menopausal women with coronary heart disease.5.In premenopausal women with coronary heart disease,the coronary artery is more common in single-vessel disease,mostly in the anterior descending branch.After menopause,the scope of coronary lesions is wide and heavy.
