| Objective To observe the clinical therapeutic effect of modified rhinoceros horn Rehmannia Decoction on children purpura nephritis with blood-heat and blood-stasis syndrome,and to evaluate the effect of modified rhinoceros horn Rehmannia Decoction on children purpura nephritis with blood-heat and blood-stasis syndrome,so as to provide scientific data for the treatment of syndrome differentiation and the application of modified rhinoceros horn Rehmannia Decoction in children Pura nephritis with blood-heat and blood-stasis syndrome,and to evaluate the effect of modified rhinoceros horn Rehmannia Decoction Clinical efficacy of improving blood urine,proteinuria and symptoms in patients with blood-heat and blood-stasis syndrome.Methods From January 2017 to June 2018,184 cases of pediatric Henoch-Schonlein purpura nephritis with blood-heat and blood-stasis syndrome were treated in our hospital.According to HSPN diagnosis and the inclusion criteria of blood-heat and blood-stasis syndrome,the course of HSPN less than 12 weeks and the consent of family members,122 cases of children who did not meet the requirements of this study were excluded according to the relevant exclusion criteria.After obtaining the consent of all the children’s familymembers and myself,the patients were divided into two groups by random number sub-table method.62 cases in the control group were treated with conventional Western medicine,and prednisolone acetate tablets,dipyridamole tablets and rutin tablets were used for clinical treatment.In the treatment group,60 cases were treated with TCM syndrome differentiation on the basis of Western medicine treatment,and modified rhinoceros horn and Rehmannia Decoction was used according to the actual disease of children.Both groups were treated for 12 weeks.The clinical symptoms and signs of the children were observed and recorded during the treatment.The curative effect was judged according to the Guiding Principles of Clinical Research of New Chinese Medicine and the integral of TCM symptoms.The clinical efficacy and side effects of Jiawei Xijiao Dihuang Decoction were determined according to the changes of proteinuria,hematuria and urinary microalbumin.Results According to the curative effect comparison of clinical comprehensive treatment,after 12 weeks of treatment,the symptoms of children in the treatment group improved significantly,and the number of cured,effective and effective cases in the treatment group was significantly higher than that in the control group.The symptoms in the control group were better than those before treatment,but the overall curative effect was obviously insufficient compared with that in the treatment group.Through statistical data analysis,the total effective rate between the twogroups had a strong difference(P < 0.05).After 12 weeks of treatment,the total score of TCM symptoms in both groups decreased,but the total score of TCM symptoms in the treatment group was significantly lower than that in the western medicine treatment group.After treatment,the total score of TCM symptoms in the two groups had statistical significance(P < 0.05).The application of TCM treatment program significantly improved TCM symptoms in children,and the curative effect was significantly better than that in the control group.The treatment group and the control group improved the scores of abdominal pain,joint swelling and pain,but the treatment group could significantly improve the scores of children’s upset,thirst,red epistaxis,constipation,etc.,while the control group had no significant effect.After 12 weeks of treatment,there were significant differences in urinary red blood cell count and urinary protein between the two groups.The 24-hour urinary protein quantification(24hUmAlb),urinary microalbumin(mAlb)and hematuria and proteinuria decreased significantly in the treatment group,and the effect was significantly better than that in the control group(P < 0.05).In addition,although there were slight adverse reactions in the course of treatment,the two groups were tolerable,and no other special symptoms appeared in the treatment group.Therefore,the treatment group was safe and had no obvious adverse side effects.Conclusion Modified rhinoceros horn Rehmannia Decoction Combined with western medicine can notonly significantly improve the discomfort symptoms of children with purpura nephritis with blood heat and blood stasis,improve the overall viscera function,effectively restore kidney function,improve the symptoms of children with traditional Chinese medicine,but also reduce urinary red blood cells and urinary protein.From the two groups of children with normal liver and kidney function,no serious symptoms and signs of discomfort,Modified Rhinoceros Horn Dihuang Decoction has no obvious side effects,has a strong safety.In clinical treatment of children with purpura nephritis of blood heat and blood stasis type,compared with simple western medicine treatment,the implementation of modified rhinoceros horn and Rehmannia Decoction Combined with western medicine treatment program can further improve the therapeutic effect of the disease.Based on the analysis of the effect of modified rhinoceros horn Rehmannia Decoction on hematuria,urinary protein and clinical symptoms after treatment of Henoch-Schonlein purpura nephritis,it can be inferred that the blood-heat stagnation type of Henoch-Schonlein purpura nephritis in children is related to inflammatory reaction and immune function,but the actual mechanism needs further study. |
