Application Research Of "Trinity" Management Of Tuberculosis Prevention And Control In Shimian County

Objective:To investigate that the “Trinity” management model can help reducing the incidence of tuberculosis(TB),drug resistance rate and cure rate of tuberculosis patients;and to explore a more effective prevention and control modelt for tuberculosis in our county.Methods : Collecting data of all tuberculosis patients before the implementation of the “Trinity” prevention and management model through CDC;Collecting data of all tuberculosis patients after implementation of the “Trinity” prevention and management model through the His system of Shimian County People’s Hospital.Patient data included gender,age,ethnicity,occupation,date of onset,sputum smear,comorbidities,complications,treatment and outcomes,etc.Data entry and statistics using Excel 2007 and SPSS 16.0,and plotted with Graph Pad Prism 5.Results: 1.“Trinity” prevention and management model helped reducing the incidence of tuberculosis and improve the cure rate of tuberculosis patients in Shimian County(P<0.05).2.The number of tuberculosis was higher in men than in women,and the ratio of male to female incidence is 1.5:1.3.The age of onset of tuberculosis had three peak periods,which are around 20 years old,40-45 years old,and 65-70 years old.The average age of tuberculosis was about 44 years old.4.80% of tuberculosis patients are employed as farmers,followed by students and other occupations(P<0.05);5.The highest number of tuberculosis was Han Chinese,followed by Yi and Tibetan.6.Some patientswith tuberculosis had silicosis or chronic bstructive pulmonay disease,and the number of patients with silicosis was higher than those with chronic bstructive pulmonary disease(P<0.05).7.Tuberculosis complications were more common with hyperuricemia and liver damage,and more patients with hyperuricemia than patients with liver damage(P<0.05).8."Trinity" prevention and management system helped to improve the positive detection rate of sputum smear.9."Trinity" prevention and management model helped reduce the number of patients with tuberculosis(F=3.85,p=0.00).10.The "Trinity" prevention and management model could help improve the drug resistance of tuberculosis patients(P<0.05).conclusion: 1.“Trinity” prevention and management model helps reducing the incidence of tuberculosis,drug resistance rate and improve the cure rate of tuberculosis patients.2.“Trinity” prevention and management model is only for the registered population and permanent residents of the county,and the management of tuberculosis for the floating population is still not in charge.
