Objective:to investigate the Histopathological Characteristics of the liver in patients with chronic hepatitis b and to screen the routine clinical biochemical indicators related to the abnormal histological lesions.To understand the influencing factors of liver pathological changes in chronic hepatitis b patients,and to provide experimental basis for clinical treatment of chronic hepatitis b patients.Methods:a total of 262 patients with hepatitis b hospitalized in our hospital from June 2015 to June 2018 were elected as study subjects to analyze the pathological characteristics of patients at different ALT levels,so as to further analyze the relationship between histoligical changes and conventional clinical biochemical indicators.SPSS 18.0 software was the data statistical analysis software used in this study,data were represented by mean standard deviation or quantity(%),accurate probability method or chi-square test method was used for inter-group counting data comparison,and Spearman was used for correlation analysis.The relationship between each parameter and the significant hepatic histological abnormality was analyzed by non-conditional logistic regression.The diagnostic value of the cross-examination was assessed using the subject work characteristic curve.Bilateral test was used for statistical test.The test standard was mainly a=0.05,and P<0.05 was considered statistically significant.Results:1.Under the different levels of ALT,262 cases of chronic hepatitis b patients with significant inflammation incidence is ALT ULN group 17.31%(9/52),or less ALT 1-2ULN group 44.21%(42/95),the ALT>2ULN group 92.12%(92/115).Under different ALT levels,the incidence of significant fibrosis as: ALT ULN group 19.23%(10/52),or less ALT 1-2 ULN group 37.89%(36/95),the ALT>2 ULN 61.74%(71/115)Results show that the incidence of significant incidence of liver inflammation and fibrosis,ALT>2 ULNgroup,all patients than ALT 1-2 ULN group and ALT ULN or less presents the result of increased significantly(P=0.001)or less.2.Univariate and multivariate analyses of the correlation between ALT,AST,Alb and significant inflammation showed that the area under the ROC curve of significant inflammation was(0.836 ~0.869 0.03)under the prediction of ALT and AST.3.Single-factor and multi-factor analyses showed that AST,PLT,AKP and AKP were related to the marked fibrosis,and the ROC curve of the hepatic marked fibrosis was predicted on a ri model consisting of PLT,AKP and GGT with an area of 0.841~0.882 0.03.Conclusion:1.n the liver histology of chronic hepatitis b patients,normal or slightly elevated ALT was relatively common.2.APAG is a noninvasive marker used to predict hbv-associated significant liver fibrosis. |
