| BACKGROUND:Pediatric chronic rhinosinusitis,(PCRS)is a common disease in the children group,the number of outpatients visits every year more than the number of patients with acute sinusitis,patients with allergic rhinitis children were flat compared with adult patients with chronic sinusitis,its epidemiological study is relatively small,children and adults may be due to physical differences such as sinus development not completely and adenoidectomy proliferous hypertrophy block the airway drainage,thus in the evaluation of curative effect for chronic sinusitis often ruled out by the researchersObjective:To investigate the therapeutic effect of balloon catheter dilation of the sinuses on children with chronic sinusitis who are not well treated with conservative medicine.And to evaluate the value of balloon catheter dilation in the treatment of children with chronic sinusitisMethods:Selectthe third affiliated hospital,southern medical university in January 2017-December 2018 to the hospital otolaryngology outpatient visits,and the inefficiency of drug treatment of patients with chronic sinusitis in children aged 4 to 12,the diagnostic reference standard of chronic nasal sinusitis diagnosis and treatment guidelines(2012,kunming),38 patients by inclusion criteria and exclusion criteria screening in experiment scheme according to the cases with parents take communication,introduces two kinds of treatments(balloon sinus surgery therapy and drug therapy)details,willing to choose one of the parents according to the treatment planAmong the 38 subjects,22 were selected to receive sinus balloon dilation.Sixteen subjects were enrolled in the drug treatment group.Preoperative to into the group of patients using validated questionnaires,by filling in the SN-5(sinusitis)quality of life scale and VAS(visual analog scale)subjective symptom assessment by Lund-Kenndy scoring(endoscopic mucosal morphology scoring system)and Lund-Mackay rating carry on objective appraisal(sinus CT grading system),to determine the baseline data after treatment for 3 months and 6 months follow-up follow-up regularly ended in December 2018,eventually 32 subjects completed 6 months of follow-up studyAnother 5 patients(3 in the surgery group and 2 in the drug group)lost follow-up due to telephone or address changes,and 1 patient was excluded because of the change from BSD to functional nasal endoscopic surgery.Results:The same stage in the two groups after treatment compared,sinus balloon expansion treatment VAS score of SN-5 a grade Lund-Kenndy Lund-Mackay rating scores dropped significantly in the medicine conservative treatment group,difference has statistical significance(P<0.05)after surgical treatment of sinus balloon expansion follow-up VAS score of SN-5 a grade Lund-Kenndy Lund-Mackay grading was a significant reduction in the baseline,the difference was statistically significant(P<0.05),In the continuing drug conservative treatment group,except that the VAS score sn-5 in the 3-month follow-up after treatment was significantly decreased compared with the baseline before treatment,the difference was statistically significant(P<0.05).The other follow-up scale scores were not significantly decreased compared with the baseline before treatment,and the difference was not statistically significant(P>0.05).Conclusion:Balloon sinus dilatation is a safe and technically feasible method for the treatment of PCRS.Patients who underwent balloon sinus dilation alone and balloon sinus dilation combined with adenoidectomy had improved clinical symptoms and quality of life during a 6-month follow-up period.Due to the limited sample size,more studies are needed in the future to more comprehensively define the role of BSD in the surgical treatment of chronic sinusitis in children. |
