Purpose:Observe the effect of Danqi Jiangtang Capsule on serum 25-(OH)-D,PTH and antioxidant index HO-1,SOD in patients with diabetic osteoporosis and the improvement of TCM symptoms The effect of Danqi Jiangtang Capsule on serum IGF-1 and T in rats,evaluate the therapeutic effect of Danqi Jiangtang Capsule on sugar and bone metabolism,and analyze the possible mechanism of action of Danqi Jiangtang Capsule on sugar and bone metabolism.It provides a new idea for the prevention and treatment of diabetic osteoporosis by Chinese medicine.Methods:1.Clinical Research: 68 patients with type 2 diabetes mellitus and osteoporosis who met the diagnostic criteria of this study were randomly divided into control group and treatment group,with 34 cases in each group.On the basis of diet,exercise and conventional hypoglycemic treatment,the two groups were treated with oyster calcium carbonate chewable tablets + Alfacalculated alcohol soft capsules in the control group;the treatment group was treated with Danshen Jiangtang Capsule on the basis of the control group.Take 4 weeks as a course of treatment,a total of 3 courses.Observed the scores of TCM syndromes,FPG,2hPG,HbA1 c,FCP,2hPCP,serum Ca,PTH,25-(OH)-D,BMD and serum HO-1,SOD in the two groups before and after treatment;Safety indicators for both groups of patients.To investigate the effects of Danqi Jiangtang Capsule on serum 25-(OH)-D,PTH and antioxidant index HO-1,SOD in patients with diabetic osteoporosis and the improvement of TCM symptoms,and evaluate the effect of Danqi Jiangtang Capsule on sugar and bone.The therapeutic effect of metabolism,analysis of the possible mechanism of action of Danqi Jiangtang Capsule on sugar and bone metabolism.2.Experimental Research:42 healthy male rats were selected and weighed 200±20g.After 1 week of normal feed feeding,they were randomly divided into normal group and experimental group by digital table method.The normal group continued to feed the common diet.After 4 weeks of high-fat diet feeding,fasting for 8 hours,streptozotocin(STZ)35mg/kg was intraperitoneally injected,and the random blood glucose was ≥16.7mmol/L.After successful modeling,the rats in the experimental group were randomly divided into 10 model groups,8 DJC high and low dose groups and 8 piglets.The normal control group was given normal diet,the experimental group continued to give high-fat diet,the normal group and the model group were given a certain amount of normal saline;the DJC high-and low-dose groups were given Danqi Jiangtang Capsules 1.08g/(kg·d),0.54g/(kg·d)was intragastrically administered;the pioglitin group was administered with pioglitazone 10 mg/(kg·d);the rats were weighed once at the 4th and 8th weekends,and the tail vein was measured at the end of the tail.Rat blood glucose;rats were sacrificed after 8 weeks,and blood was taken from the abdominal aorta for detection of serum levels of IGF-1 and T.Results:1.Clinical results: After 12 weeks of treatment,the total effective rate of TCM syndromes in the treatment group was 87.50%,which was significantly better than the total effective rate of the control group(64.52%)(P<0.05).After treatment,the TCM symptoms of the two groups were improved compared with before treatment.The improvement of the group was better than that of the control group(P<0.05),especially the dryness of thirst,fatigue,weakness of the waist and knees,and improvement of waist and leg pain.Compared with before treatment,the total scores of TCM syndromes of the two groups after treatment were all Decreased(P<0.01);after treatment,FPG,2hPG and HbA1 c decreased in the two groups(P<0.01);the treatment group was superior to the control group(P<0.05);after treatment,the FCP and 2hPCP were higher than the treatment in the two groups.Before(P<0.01);and the FCP of the treatment group was significantly higher than that of the control group(P<0.05);the serum levels of 25-(OH)-D were increased in the two groups(P<0.01);the serum PTH was decreased(P<0.01).The PTH of the treatment group was better than that of the control group(P<0.05).After treatment,the serum levels of HO-1 and SOD were increased in the two groups(P<0.01).Compared with the control group,the above indicators were significantly higher than the control group.The group(P<0.05);the BMD of the two groups increased after treatment(P<0.05 or P<0.01);and the treatment group BMD increased more significantly(P <0.05).2.Animal experiment results: Compared with the normal group,the blood glucose of the model group was significantly increased(P<0.01).After 4 weeks and 8 weeks of treatment,the blood glucose levels of the DJC high and low dose groups and the Pygge group were lower than those of the model group(P<0.05 or P<0.01);Compared with the normal group,serum IGF-1 and T levels in the model group were significantly lower(P<0.01);after 8 weeks of treatment,compared with the model group,DJC high and low dose groups and the pig serum group IGF-1 and T levels were significantly elevated(P < 0.05 or P < 0.01).Conclusion:Danqi Jiangtang Capsule significantly improves TCM syndrome scores in patients with diabetic osteoporosis and relieves clinical symptoms.Its mechanism may be related to lowering blood sugar,improving insulin resistance,promoting 25-(OH)-D synthesis,inhibiting PTH secretion,and increasing Serum HO-1 and SOD levels are related to the body’s antioxidant capacity.Animal experiments confirmed that Danqi Jiangtang Capsule can lower blood sugar and increase serum IGF-1 and T content,thereby promoting bone formation of osteoblasts and inhibiting bone resorption of osteoclasts.
