Study On The Intervention Mechanism Of Dan Zhi Jiangtang Capsule On Diabetic Angiopathy Based On DAG/PKC/NF-kappa B Pathway

Objective:Taking the method of Supplementing Qi,nourishing yin and activating blood circulation to dredge collaterals,based on the DAG/PKC/NF-kappa B pathway,we discussed the intervention mechanism of Dan Zhi Jiang Tang Capsule on diabetic angiopathy patients and the protective effect on vascular endothelium of model rats.Method:1 clinical study Methods in October 2015-2017 April,100 patients with early diabetic angiopathy diagnosed in the First Affiliated Hospital of Anhui University of traditional Chinese medicine were randomly divided into the control group and the treatment group,50 cases in each group.Basic treatment: all patients received basic hypoglycemic,lipid and other treatment and diabetes diet education.The control group was treated with cilostazol.For 12 weeks,fasting glucose were recorded before and after treatment(FPG),2 hour postprandial blood glucose(2h PG),glycosylated hemoglobin(Hb A1c),total cholesterol(TC),triglyceride(TG),low density lipoprotein(LDL-C),high density lipoprotein(HDL-C),transcutaneous oxygen pressure(Tc PO2),ankle brachial index(BMI),DAG,PKC,NF-index kappa B TCM syndrome score,using statistical methods,compare the difference before and after treatment of.2 Experimental Research A total of 60 SD rats were fed for 1 weeks.According to the random number table,they were randomly divided into blank group,model group,high dose group,middle dose group,low dose group and pioglitazone group.The rats in experimental group SD rats were fasted for 18 h,with a single intraperitoneal injection of STZ(STZ),the ratio of 35mg/kg,were collected from the tail vein of fasting blood glucose determination,fasting blood glucose than 16.7mol/L successful model signs.The rats with diabetic lower limb ischemia models survived 48.According to the random number table method,they were randomly divided into model group and pioglitazone group,and the 5 groups were high,medium and low group,8,10,10,10 and 10,respectively.The rats in the high dose group were gavaged with 0.66g/kg and D.The rats in the low dose group were gavaged with 0.17g/kg and D.The western medicine group was given pioglitazone 10mg/kg D.The rats in the blank group and the model group were gavaged with 0.9% normal saline 5ml/kg / D,and once a day,the time of administration was 12 weeks.After 12 weeks of administration,all rats fasted 12 h and weighed and detected the blood glucose at the tip of the tail.After abdominal anesthesia,the abdominal aortic blood 8ml was extracted and placed in the EP tube.Result:1.clinical observation results The clinical observation showed that after 12 weeks of treatment,the treatment group and the control group symptom total effective rate were 93.75% and 72.91%,the difference was statistically significant(P<0.05);after the treatment of TCM syndrome integral of the two groups were significantly decreased than before treatment,TCM symptom and treatment group improved better than those in the control group,the difference was statistically significant(P < 0.05);the two groups after treatment of FPG,2h PG and Hb A1 c were lower than before treatment(P<0.01),compared with the control group,the treatment group decreased significantly(P<0.05);the two groups after treatment TG,TC and LDL-C levels than before treatment decreased(P < 0.05),HDL-C level(P < 0.05),the difference was statistically significant.Compared with those before treatment,the two groups were significantly reduced in TCM symptom scores and Toronto scores after treatment(P < 0.01),nerve conduction velocity was improved(P<0.01),and the treatment group was better than the control group.The difference between the two groups was statistically significant(P<0.05).The content of serum beta-EP and NGF in the treatment group and the control group was significantly higher than before the treatment(P<0.05),and the treatment group was significantly better than the control group(P<0.01).2 animal experiment results Animal experimental results show that compared with the control group,the rats in the model group,DAG PKC and NF-K B were significantly decreased,P<0.01;compared with the model group,high and low dose traditional Chinese medicine group DAG,PKC,NF-kappa B increased the level of P<0.01;high dose group and low dose group rats,DAG PKC,NF-kappa B,P<0.05.Compared with the blank group,the expression of NGFm RNA in the model group was significantly decreased(P<0.01),and the expression of NGFm RNA in the high and low dose group was higher than that in the model group(P<0.05).Conclusion: Dan Zhi Jiangtang Capsule may effectively improve patients with Qi and yin deficiency syndrome.Decrease fasting blood glucose,2h postprandial blood glucose and glycated hemoglobin level,blood lipid metabolism regulation,oxidative stress response,prevention and treatment of delayed vascular atherosclerosis formation and development;increased in patients with ABI,Tc PO2 levels in peripheral tissues,improve microcirculation;elevated serum DAG,PKC,NF-kappa B level,improving oxidative stress.Dan Zhi Tang Tang Capsule can reduce blood glucose and blood lipid level,reduce the expression of DAG/PKC/NF-kappa B,inhibit the inflammatory reaction and oxidative stress reaction of ischemic tissue,and slow down the formation of atherosclerotic plaques.
