Research background and purpose: Liver cirrhosis is one of the most common diseases in the job of digestive system department.There are a lot of complications of liver cirrhosis,portal vein thrombosis is one of that,including the thrombosis from main portal vein,Left or right branch of portal vein,splenic vein and superior inmesenteric vein.A lot of studies have showed that PVT improves the possibility of other complications and increase mortality rate,therefore,we should pay more attention to it.It is difficult for doctors to prevent and treat PVT because there are little symptoms in the development of PVT.The paper about risk factors of PVT in patients with liver cirrhosis are controversial and there is no certain results.In this study,we do a research about general conditions,related diseases and blood index of patients attacked by liver cirrhosis,in order to explore the pathogenesis of PVT and offer references to clinical jobs.Materials and methods: We selected 245 patients with liver cirrhosis who went to the first Hospital of Jilin University for treatment from January 2013 to October 2018 as our subjects.Inclusion criteria: the patients with decompensated liver cirrhosis according with guideline of diagnosis and treatment were included,125 patients with PVT should have certain imaging results.Exclusion criteria: the patients with other diseases or under some certain treatment which has a influence to the blood index.Grouping: the patients with PVT acted as group A,the patients without PVT acted as group B.We collect data of general conditions,related diseases,blood routine examination,Coagulation function examination and imaging tests to do statistical analysis within SPSS22.0.Results: 1、There are a total of 246 patients(144 males and 102 females),group A has 121 patients(65 males and 56 females),group B has125 patients(79 males and 46 females),the difference has no statistically significance(P>0.05).The average age of group A is 53.42±10.991,the average age of group B is 53.45±10.625,the difference has no statistically significance(P>0.05).There are no significant difference in proportion of patients who have diabetes between two groups.Group A has a higher rate of splenectomy than group B,the difference has no statistically significance(P≤0.05).2、The major types of liver cirrhosis are,hepatitis B cirrhosis,hepatitis C cirrhosis,alcoholic cirrhosis,drug-induced cirrhosis,primary biliary cirrhosis,anto-immune cirrhosis,liver fluke cirrhosis,crytogenic cirrhosis,the rate of pathogenesis has no statistically significance between two groups(P>0.05).3、The level of HGB,PLT,PCT in group A is higher than group B,the difference is statistically significant(HGB groups P=0.02,P≤0.05;PLT groups P=0.003,P≤0.05;PCT groups P=0.003,P≤0.05).The level of MPV and PDW has no significant difference between two groups(P>0.05).4、The level of TT and APTT in group A is lower than group B,the difference is statistically significant(TT groups P=0.00,P≤0.05;APTT groups P=0.017),The level of FBG in group A is higher than group B,the difference is statistically significant(APTT groups P=0.029,P≤0.05).The level of PT,INR,PTR and PTA has no significant difference between two groups(P>0.05).5、PVT include the thrombosis from main portal vein,Left or right branch of portal vein,splenic vein and superior inmesenteric vein.PVT involving single branch:25 patients,two branches:34 patients,three branches: 37 patients,four branches 18 patients,five branches:7 patients.In conclusion: 1、The history of splenectomy improve the risk of PVT in liver cirrhosis.The patients who have the spelenectomy should get positive supervision to avoid the occurrence of other complication.2、The increase of HGB,PLT and PCT has a influence to the formation of PVT,and indicate the increase of blood viscosity.Those patients with the features are inclined to have thrombosis.3、The patients attacked by PVT with liver cirrhosis has a low level of TT and APTT and a high level of FBG.That means the patients are in a stage of hypercoagulability which could be the reason of PVT.4、The history of splenectomy,increased HGB and decreased TT are the independent risk factors for PVT formation.
