ObjectiveStudy oral polyethylene glycol electrolyte powder method,open plug dew enema method,natural emptying method in three different ways for intestinal preparation used for hemorrhoids perioperative clinical curative effect and security,through the observation and comparison of three different bowel preparation method for hemorrhoids patients perioperative adverse reaction in the process of bowel preparation,intraoperative intestinal cleanliness,postoperative complications,such as evaluation of the pros and cons of the three different schemes for intestinal preparation,solution for intestinal preparation for hemorrhoids patients perioperative optimized choice,for hemorrhoids perioperative surgical support rapid recovery.MethodsA total of 129 patients diagnosed with mixed hemorrhoids and dialectically classified as wet-heat type were collected from the inpatient department of guangdong hospital of traditional Chinese medicine from October 2018 to February 2019.They were randomly divided into the oral polyethylene glycol electrolyte powder group(group A),the kaiseru enema group(group B)and the natural emptying group(group C),with 43 patients in each group.Group A was treated with polyethylene glycol electrolyte powder 1(68.56g)and 2000ml warm water in the evening before surgery.Group B was given 100ml of kaiseru+0.9%sodium chloride 500ml enema;Patients in group C were asked to empty their stool in the evening or morning before operation.To observe the patient’ s tolerance during perioperative intestinal cleansing,the number of bowel movements during intestinal cleansing,the effect of intraoperative intestinal cleansing,postoperative complications,and postoperative first bowel movements;Adverse reactions during intestinal cleansing were recorded in the three groups.In terms of statistics,SPSS22.0 statistical software was used for data analysis according to the efficacy evaluation criteria,and the safety and effectiveness of the three schemes were compared.Resulis:1.The baseline indicators of patients in the three groups were consistent in terms of age,gender,course of disease,surgical method,number of surgical incisions,and hospitalization cost,with no statistical significance(P>0.05).2.In terms of length of stay,the average length of stay of patients in group A was 4.88 1.62 days,the average length of stay of patients in group B was 4.14 1.73 days,and the average length of stay of patients in group C was 3.95 1.25 days.The comparison among the three groups showed statistical difference(P<0.05),and the number of days of stay of patients in the natural emptying group was even less.3.In terms of adverse reactions in the process of intestinal cleaning in the three groups,no nausea,vomiting or dizziness occurred in the three groups,and no abdominal pain or abdominal distension occurred in the patients in group A and group B.There was no statistical significance among the three groups.There were 3 patients in group A whose sleep was affected,while there were 0 patients in the other two groups(P<0.05).In terms of the time required for the whole intestinal cleaning and the frequency of defecation in the process of intestinal cleaning,the comparison between the three groups was statistically significant,with group C having the lowest time required and the lowest frequency of defecation.For patients’ subjective feelings,the tolerance rate of group A was 88.4%,group B was 90.7%,and group C was 100%.There was no statistical difference in P>0.05.Overall,there were fewer complications and higher patient comfort and acceptance in group C among the three groups.Both group A and group B need A lot of preparation and education in nursing before intestinal cleaning,thus group C indirectly reduces the workload of nursing and medical treatment.4.There were statistically significant differences in intraoperative intestinal cleanliness between the three groups(X2=8.014,P=0.016).Seven patients in the oral polyethylene glycol group did not meet the requirements for surgery,and four patients in the open enema group did not meet the requirements for surgery.Compared with the three groups,the operation time of the natural emptying group was the least.There is no intraoperative time required to deal with the overflow of feces,so it takes the least time on average.Visible bowel cleanness is better,the operation is smoother,time-consuming less.5.For postoperative complications,there were no statistically significant differences in bleeding,anal edema,urinary retention and intraoperative infection;Postoperative pain was statistically different among the three groups,with the lowest mean score in the natural defecation group.The first postoperative defecation was the best in the natural emptying group,and the difference was statistically significant.Conclusion:Oral polyethylene glycol electrolyte powder,open enema and natural emptying method are three methods of bowel cleaning before hemorrhoids surgery.The natural emptying group is more comfortable,has fewer days of hospitalization,has a high qualified rate of intraoperative intestinal cleaning,has the least operation time,reduces postoperative pain,and improves postoperative satisfaction with first bowel movement.In general,the natural emptying method is safe and reliable,which can reduce the number of days in hospital and accelerate the operation of the department.It is worthy of clinical promotion,reduces the workload of medical care,and conforms to the concept of rapid recovery during the perioperative period of hemorrhoids.
