ObjectivePeripheral facial paralysis is a common peripheral neuropathy,its incidence and recurrence rate increased year by year,there is no unified clinical treatment standard.The purpose of this study was to observe the clinical efficacy of different stages of peripheral facial paralysis treated by hou-qi acupuncture.The objective data monitored by laser speckle contrast imaging technology and the evaluation of facial disability index on the body function and social activities of patients with facial paralysis were used to objectively analyze the physical and mental effects of hou-qi acupuncture on patients with peripheral facial paralysis.MethodsIn this study,60 patients with peripheral facial paralysis who met the inclusion criteria were randomly divided into acute phase group,resting phase group and convalescent phase group,each with 20 patients.The three groups were given the same treatment of hou-qi acupuncture once a day for 25min each time for 10 consecutive days.each one should be in a constant temperature(24±26℃)environment,through laser speckle monitoring instruments monitoring the blood perfusion of facial specific ROI area(with the mouth,the healthy side of the mouth,with side cheeks,the healthy side cheek).This study evaluated the therapeutic effect of hou-qi acupuncture before the first treatment and after the 10th treatment.The main outcome Index was House-brackmann scale,and the secondary outcome Index was FDI Disability Index and microcirculation perfusion.In this study,spss21 was used for statistical analysis to compare the differences in the effective rate and recovery rate of H-B scale、FDI scale and PU in different stages.Results1.Efficacy evaluation of hou-qi acupuncture for patients with peripheral facial paralysis at different stages(1)60 patients were included in this study,with a total of 18 patients cured and a cure rate of 30%;there were 25 cases and the efficiency was 41%.14 cases were effective,and the effective rate was 23%.There were 3 invalid cases and the efficiency was 5%.(2)A total of 20 patients in the acute stage group were cured,with a recovery rate of 45%.There were 8 cases with significant efficiency,and the efficiency was 40%.The effective number was 3,and the effective rate was 15%.There were O invalid cases and 0%inefficiency.(3)A total of 20 patients were in the resting stage group,and the number of cured patients was 5,with a recovery rate of 25%.There were 10 cases with significant efficiency,50%with significant efficiency.The effective number was 3,and the effective rate was 15%.Two cases were invalid,and the efficiency was 10%.(4)In the convalescent group,there were 20 cases in total,with 4 cases recovered and the recovery rate was 20%.There were 7 cases with significant effect,and significant efficiency was 35%.8 cases were effective,and the effective rate was 40%.Invalid number 1 case,inefficient 5%.In this study,the total effective rate of hou-qi acupuncture on patients with different diverging peripheral facial paralysis was up to 95%,indicating that hou-qi acupuncture had a therapeutic effect on peripheral facial paralysis.The cure rate of the acute stage group was higher than that of the rest stage group and the convalescent stage group.The significant efficiency in the resting stage was higher than that in the acute stage and the convalescent stage.The effective rate in the convalescent group was higher than that in the acute stage and the rest stage.The results indicated that the therapeutic effects of different stages of treatment with the hou-qi acupuncture were different,and the earlier the treatment with the hou-qi acupuncture for peripheral facial paralysis,the better the therapeutic effects.2.Comparison of the blood perfusion of peripheral facial palsy in different stages by hou-qi acupuncture(l)The hou-qi acupuncture has an effect on the blood perfusion volume(PU)of peripheral facial paralysis at different stages.After treatment with hou-qi acupuncture,both the healthy side and the affected side had statistical significance(P<0.05).(2)There was a statistical difference in the measurement of blood perfusion of the affected cheek and the healthy cheek in the self-before and after the treatment of acute syndrome by acupuncture(P<0.05).There was no statistical significance in the perfusion volume of the healthy side and the affected side(P>0.05).(3)The blood flow of the affected side of the mouth,the uninjured side of the mouth,the affected side of the cheek and the uninjured side of the cheek was increased in the control group before and after the treatment with the hou-qi acupuncture in the stationary stage.The blood perfusion volume of the affected side of the mouth,the uninjured side of the mouth and the affected side of the cheek was statistically different(P<0.05).There was no statistical significance in the blood perfusion volume of healthy lateral cheek(P>0.05).(4)In the recovery phase group,the blood perfusion volume of healthy side cheeks was statistically significant(P>0.05).The blood perfusion volume of the affected cheek,affected mouth and healthy mouth showed no statistical significance(P<0.05).The blood perfusion volume of peripheral facial paralysis patients was increased before and after the treatment with hou qi acupuncture.The functional sites of different divergences were different,the blood flow of the healthy side increased in the acute phase,the blood flow of the healthy side mouth and the healthy side increased in the stationary phase,and the blood flow of the healthy side increased in the recovery phase.Therefore,the laser speckle contrast imaging technology can be used as an instrument to monitor the improvement of facial microcirculation,and can also be used to judge the prognosis of peripheral facial paralysis.Whether this technology can be used to quantify the development of peripheral facial paralysis remains to be confirmed by more experimental studies.3 The FDI scale(FDIP scale and FDIS scale)of the patients with peripheral facial paralysis at different stages were compared by hou-qi acupuncture method(1)Hou-qi stitch on facial paralysis disabled index(FDIP)physiological function assessment scale of comparison:Three groups after treatment FDIP rating scale has statistical significance(P<0.05),the two analysis it is concluded that the acute phase and stationary phase group,recovery and acute phase group had no statistical significance(P<0.05),the stationary phase and recovery groups have statistical significance(P>0.05).In the further analysis of the sub-items of the FDIP scale,the food score was statistically significant(P<0.05).This indicated that the hou-qi acupuncture improved the score of FDIP scale,especially the difficulty of eating.(2)Comparison of the social function index(FDIS)evaluation scale of the facial paralysis disability index(FDIS)by the method of hou-qi acupuncture:after treatment,the FDIS scale score of the three groups showed no statistical significance(P>0.05).Pair-based analysis showed that there was no statistical significance between the acute stage group and the rest stage group,the recovery stage group and the acute stage group,and the rest stage group and the recovery stage group(P<0.05).After in-depth analysis of the sub-items of FDIP scale,the scores of quiet time,isolation time and social activity were statistically significant.It indicates that the hou-qi acupuncture can improve the score of FDIS scale,and it is especially good at improving social communication activities and personal emotional changes.It emphasizes the importance of traditional techniques and has a certain therapeutic effect on different stages of peripheral facial paralysis.Combined wi th the above scale and objective indications,it can not only improve the dysfunction caused by facial paralysis,but also alleviate the negative emotions of patients.This shows that the role of the "hou qi" acupuncture is not only local,but also attaches importance to the overall regulation,which is in line with the overall view of traditional Chinese medicine and the theory of treatment based on syndrome differentiation.Further basic experimental research is needed on the specific mechanism and system of the effect of climatic acupuncture.Conclusion1.Hou-Qi needling therapy has a certain therapeutic effect on different stages of peripheral facial paralysis,and the effect of interventional needling in acute stage is more obvious.2.Hou-Qi needling therapy emphasizes "Shou-shen" "Shou-ji",which can improve facial paralysis dysfunction and alleviate patients’ negative emotions.The specific mechanism and mechanism of climatic needling still need further basic experimental research.3.Laser speckle contrast imaging can be used as an instrument to monitor the improvement of Facial Microcirculation and to judge the prognosis of peripheral facial paralysis.Whether this technique can be used to quantify the development of peripheral facial paralysis remains to be confirmed by more experimental studies.
