| Objective:To explore the correlation between intravenous glutamine supplementation and postoperative intestinal mucosal barrier function recovery in patients with colon cancer,so as to provide a basis for intravenous supplementation of patients with colon cancer after surgery.Methods:The clinical data of 98 patients who underwent colon cancer surgery in XXX from May 2016 to August 2017 were collected.There were 46 patients in the observation group,including 29 males and 17 females with an average age of56.70±7.56 years old;52 patients in the control group,23 males and 29 females,with an average age of 53.94±8.94 years old.The bowel preparation was started 1 day before surgery.It is forbidden to take other solid foods(including rice,vegetables and soup)except drinking water.Etc.),both groups of patients underwent intravenous nutrition before surgery.The nutritional supplements were calculated according to the calorie of 25kcal/kg/d.No glutamine was added to the intravenous supplement.On the day of surgery,all patients were fasted and watered.Mirror assisted(left/right half/horizontal/sigmoid)colon cancer(enlarged)radical surgery procedure.On the first day after surgery,parenteral nutrition was started.The patients in the observation group were given alanyl glutamine injection from the first day after surgery,but not in the control group.The postoperative nutritional supplement was 25kcal/ kg / d,the mode of administration is enteral nutrition(EN)plus parenteral nutrition(PN),payattention to electrolyte and nutrient balance,start to add vegan at 48 h after surgery,according to the patient’s gastrointestinal tolerance gradually added The amount was changed to ordinary liquid food 7 days after surgery.Peripheral blood serum from the first day before surgery,the first day after surgery,the third day after surgery,the fifth day after surgery,and the seventh day after surgery were measured using the "JY-DLT Gut Barrier Function Biochemical Index Analysis System".In the "JY-DLT intestinal barrier function biochemical index analysis system",the quantitative reaction was detected by the reaction of diamine oxidase,DAO,endotoxin and reagent at 48 °C.Results The two-factor repeated measures analysis of variance,the pairwise comparison between groups was the LSD-t test,and the time-two comparison was the difference t-test(the Bonferroni adjustment was not considered for the time being).The comparison between groups of partial count data is a chi-square test.The test level α = 0.05,when P < 0.05 was statistically significant.Results:The serum DAO,D-lactic acid and endotoxin concentrations in the two groups were statistically significant(P<0.05).There was a significant difference between the overall comparison between time points and the interaction between grouping and time(P<0.05).There were no significant differences in serum DAO,D-lactic acid and endotoxin concentrations between the two groups before and after the first day after surgery.DAO and serum endotoxin began to appear statistically significant on the 5th day after surgery(P < 0.05),while D-lactic acid began to be statistically significant on the 3rd day after surgery(P < 0.05).The three indicators of the two groups were significantly different at each subsequent time point and the T1 time point.The serum DAO,D-lactic acid and endotoxin concentrations in the two groups were significantly higher than those before surgery.With the recovery of the postoperative condition,the indexes of the two groups began to decline,but the decline of the observation group in each time period was greater than In the control group,the changes of the three groups of the two groups were significantly statistically significant(P<0.05).There was no significant difference in serum DAO and endotoxin levels between the control group and the observation group before and after the first day and the third day.The changes on the 5th and 7th days began to appear statistically significant(P <0.05),D-lactic acid was statistically significant from day 3(P < 0.05).Conclusion:1.Surgery can cause damage to the intestinal mucosal barrier function of colon cancer patients.2.Intravenous supplementation of glutamine(Gln)in patients with postoperative colon cancer is beneficial for the recovery of intestinal mucosal barrier function. |
