Objective:To investigate the influence of physiological and pathological conditions at birth of newborn and gestational conditions on plasma fibrinogen degradation products(FDP)and D-dimer levels. Methods: During the study period, blood samples were collected from neonates admitted to the neonatal intensive care unit(NICU)of Tongji Medical College Affiliated Hospital of Huazhong University of Science and Technology within 2 hours after birth. The levels of FDP and D-dimer were measured and recorded.Meanwhile, collecting detailed clinical data of the child, including gender, gestational age, birth weight, the relationship between birth weight and gestational age, mode of delivery, asphyxia at birth, arterial blood p H,prenatal hormonal use,and maternal prenatal the use of anticoagulant drugs, maternal perinatal risk factors and prenatal D-dimer values.Statistical analysis was performed using SPSS 19.0 Simplified Chinese version of statistical software to compare neonatal plasma FDP and D-dimer concentrations between different genders, gestational age, birth weight, gestational age, mode of delivery, presence or absence of asphyxia, acidosis, prenatal use of hormones, and use of anticoagulation drugs, perinatal risk factors(pregnancy hypertensive disease, premature rupture of membranes, placental abnormalities, gestational diabetes). And multivariate linear regression was used to analyze the relationship between clinical characteristics and plasma concentration of FDP and D-dimer.Results: The plasma FDP and d-dimer values of 222 neonates were skewed at birth, with median values of 6.00 ug/ml and 1.74 mg/L FEU,and quartile distances of 10.40ug/ml and 2.55 mg/L FEU,respectively. The concentrations of FDP in neonates born to natural labour and cesarean section were 11.70 ug/ml and 5.30 ug/ml,respectively,and D-dimer concentrations were 2.92 mg/L FEU and 1.52 mg/L FEU,respectively. The FDP and D-dimer levels were higher in the former(Z value-4.198,P < 0.05).The levels of FDP and D-dimer in newborns with different gestational age, different birth weight and different blood p H values were compared respectively, and the differences were statistically significant(P < 0.05),two further comparison, the levels of FDP and D-dimer in neonates with gestational age < 34 weeks were significantly higher than those in ? 34 ~ < 37 group and ? 37 weeks group, the birth weight < 1500 g was significantly higher than those in ? 1500 ~ < 2500 g and ? 2500 g group, and the p H < 7.20 was significantly higher than that in the group ? 7.35.The results of multiple linear regression showed that the concentration of FDP and D-dimer in plasma was related to fetal age and arterial p H value. Conclusion: The plasma D-dimer level at birth is related to gestational age.Meanwhile, the p H level of arterial blood will also affect the value of FDP and D-dimer.The lower the p H value, the higher the concentration. The levels of plasma FDP and D-dimer at birth are not affected by gender, birth weight, relationship between birth weight and gestational age, mode of delivery, prenatal use of hormones and anticoagulant drugs, pregnancy gestational complications, the effects of asphyxia at birth and prenatal D-dimer levels.
