Objective:Uric acid has two opposite effects on promoting oxidation and anti-oxidation in different environments.Through the study of serum uric acid(SUA)level and short-term prognosis of patients with acute ischemic stroke(AIS),this study clarifies whether SUA plays an oxidative role in the acute stage of ischemic stroke,leading to cerebral ischemia aggravation,or plays an antioxidant role to protect brain tissue,in order to guide clinical intervention of SUA.Methods:1.Study subjects:Eighty AIS patients were hospitalized from August 2016 to July 2018 in the Southern Hospital of Neurology,Affiliated Hospital of Qingdao University.2.Research methods:(1)Collection of data:including the general information of the patient's past medical history,and the results of SUA and fasting blood sugar tests on the next day of admission;the criteria of hyperuricemia were:male SUA>420 umol/L,female SUA>360 umol/L.(2)Score:The patients were scored by the National Institute of Health stroke scale(NIHSS)on admission day,Modified Rankin Scale(MRS)on discharge day,and the patients with MRS score<2 were scored for good prognosis,while those with MRS score<2 were scored for poor prognosis(3)Grouping:According to SUA level,they were divided into hyperuricemia group and normal uric acid group;according to MRS score,they were divided into good prognosis group and bad prognosis group.3.Statistical methods:Statistical software SPSS24.0 was used to analyze the data.Among them,the measurements were expressed as mean±standard deviation(`x±s),and t-test was used for comparison between the two groups.The counting data were expressed as positive cases and percentage(%)and the comparison between the two groups was performed by thec~2test.Logistic regression analysis was used for multivariate analysis.P<0.05 was statistically significant.Results:1.There were significant differences in the NIHSS score,history of hypertension,history of smoking,low density lipoprotein and hyperuricemia between the group with good prognosis and the group with poor prognosis(P<0.05),but there were no significant differences in age,history of diabetes,systolic and diastolic blood pressure(P>0.05).2.There was a significant difference in the incidence of poor prognosis between hyperuricemia group and normal uric acid group(P<0.05).The percentage of good prognosis in normal uric acid group was 63.8%,which was higher than 36.4%in high uric acid group.3.Logistic regression analysis showed that the risk of adverse short-term prognosis of acute stroke was increased in patients with high uric acid,history of hypertension and high LDL,with OR values of 2.952,3.43 and 3.141,respectively.4.Logistic regression analysis showed that hypertension,hyperglycemia and dyslipidemia were not associated with the risk of hyperuricemia(P>0.05).Conclusions and significance:1.History of hypertension,high LDL and hyperuricemia are risk factors affecting short-term prognosis of AIS patients.2.Hyperuricemia,hypertension history and high LDL are independent risk factors for short-term poor prognosis of AIS patients.3.For high SUA in patients with acute cerebral infarction,clinicians should intervene and reduce SUA in time by adjusting patients'diet or medication to avoid affecting patients'prognosis.
