objective Comparison of clinical effects of modified Stoppa approach and INFIX fixation for anterior pelvic ring fractures.Methods Retrospective analysis of 79 cases of anterior pelvic ring fracture treated in the third Hospital of Hebei Province from August 2015 to February 2018,and they were divided into INFIX group(n = 40)and modified Stoppa plate fixation group(n = 39)according to different surgical methods.There were 40 cases in INFIX group and 39 cases in modified stoppa group.In INFIX group,there were 21 males and 19 females,the average age was 41.85 ±13.60.the causes of injury were as follows: falling injury in 4 cases,car accident injury in 23 cases,other 13 cases,injury score(ISS): 24-34,average 29;Fracture types(according to tile classification): type B in 31 cases,type C in 9 cases;The time from injury to operation: 5-9 days,with an average of 7 days.In the modified Stoppa approach plate fixation group,there were 29 males and 10 females,the average age was(43.74 ±14.12)years,the causes of injury were falling injury in 4 cases,car accident injury in 16 cases,other 19 cases,injury score: 21 ±29,mean 24;Fracture types: type B in 28 cases,type C in 11 cases;The time from injury to operation: 4-9 days,with an average of 5 days.The perioperative related indexes(operation time,intraoperative blood loss,fracture reduction quality(MATTA score),postoperative function score(Majeed score)and postoperative complications)were compared between the two groups.Results there were significant differences in operation time and intraoperative blood loss between the two groups,and the excellent and good rates of Matta score in INFIX group and modified Stoppa approach group were 72.50% and 84.62%,respectively,and there was no significant difference between the two groups.However,the number of excellent cases in the modified Stoppa approach plate fixation group was significantly higher than that in the INFIX group,and there was no significant difference in the excellent and good rate of Majeed score between the two groups,but there was significant difference in the total score between the two groups.There was significant difference in the incidence of postoperative complications between the two groups.Conclusion INFIX is a minimally invasive treatment with short operation time and less bleeding,but there are many complications,and the reduction of fracture in the modified Stoppa approach plate fixation group is better.
