The Effect Of Pregnancy With The Second Child On Psychological Status And Quality Of Nursing Work Life Among Nurses

Objectives In order to provide a theoretical basis for the targeted psychological intervention in pregnant nurses with the second child,and scientific,reasonable human resource allocation for nurse managers,this study intends to investigate the prevalence of depression,anxiety and quality of nursing work life,analyze influential factors of depression,anxiety and quality of nursing work life,and probe into relations of depression to quality of nursing work life,anxiety to quality of nursing work life.Methods A total of 287 nurses with the second child who came from 11tertiary hospitals in Guangdong Province were recruited by convenience sampling during September 2017 to September 2018.The Self-rating Depression Scale,the Self-rating Anxiety Scale and Quality of Nursing Work Life Scale were used to investigate the occurrence of depression,anxiety and quality of nursing work life in nurses with the second child during the gestation period.The general information questionnaire were used to analyze and compare the factors affecting depression,anxiety and quality of nursing work life in nurses with the second child during the gestation trimester.In additional,the correlations of depression to quality of nursing work life and anxiety to quality of nursing work life were analyzed.The collected questionnaires were numbered.The data was obtained by double entering using Epidata version 3.1 software.Statistical analysis was performed on the data using SPSS 21.0 software.The difference was considered statistically significant at P<0.05.Results A total of 287 pregnant nurses with the second child were included in the study.The incidence of depression was 45.6%,and the incidence of anxiety was 39.7%.Binary linear regression analysis showed that work department,night shift feeling in the early pregnancy（whether to accept night shift）,the gender of the second children excepted by themselves,the family economic pressure after the second child’s pregnancy,the level of the concern from the colleagues and superiors,and the attitude of the first child to the arrival of the second child were the main influencing factors of depression in nurses during their gestation period（P<0.05）.With or without pregnancy-related diseases,the family economic pressure after the second child’s pregnancy,the level of the concern from the colleagues and superiors were the main influencing factors of anxiety in nurses during their gestation period（P<0.05）.Among the 287 nurses,the score of QNWLS was（174.90±24.61）,and the scores of each dimension were work design（43.57±5.29）,world context（18.61±3.37）,work life/home life（25.60±5.13）,and work world（87.12±14.34）,representively.The results of multiple linear analysis showed that night shift feeling in the early pregnancy（whether to accept night shift,whether it is helpless for night shift）,the family economic pressure after the second child’s pregnancy,the level of the concern from the colleagues and superiors,and the attitude of the first child to the arrival of the second child were the factors of quality of nursing work life.The depression score of pregnant nurses with the second child pregnancy was negatively correlated with the quality of nursing work life and the scores of each dimension（r=-0.439,r₁=-0.368,r₂=-0.355,r₃=-0.467,r₄=-0.367,P<0.05）.The anxiety scores of pregnant nurses with the second child were negatively correlated with quality of nursing work life and the scores of each dimension（r=-0.308,r₁=-0.304,r₂=-0.406,r₃=-0.283,r₄=-0.357,P<0.05）.Conclusions 1.Pregnant nurses with the second child have a higher incidence of depression and anxiety,which needs to attract the attention of nursing managers.2.Work department,night shift feeling in the early pregnancy（whether to accept night shift）,the gender of the second children excepted by themselves,the family economic pressure after the second child’s pregnancy,the level of the concern from the colleagues and superiors,and the attitude of the first child to the arrival of the second child were the main influencing factors of depression in nurses during their gestation period.3.With or without pregnancy-related diseases,the family economic pressure after the second child’s pregnancy,the level of the concern from the colleagues and superiors were the main influencing factors of anxiety in nurses during their gestation period.4.Although with a high level of the quality of nursing work life,the more severe the anxiety and depressive symptoms of the pregnant nurse with the second child,the worse the quality of the nursing work life.5.According to psychological charateristics and prevalence of quality of nursing work life,nursing managers should take some appropriate measurements.
