| Objective:To observe the clinical efficacy and safety of Xiangsha Liujunzi Decoction combined with R-CHOP regimen in the treatment of B-cell non-Hodgkin’s lymphoma Spleen Stomach Qi Deficiency Syndrome,compare the changes of T lymphocyte subsets and NK cell levels before and after treatment,and then to investigate the effects of taking Xiangsha Liujunzi Decoction on chemotherapy symptoms and immune function in patients with malignant lymphoma.Methods:Fifty-six patients with B-cell non-Hodgkin’s lymphoma who were admitted to the Department of Hematology of the People’s Hospital of Fujian University of Traditional Chinese Medicine from July 2016 to December 2018 were enrolled.TCM syndromes were all syndromes of Spleen Stomach Qi Deficiency.56 patients were divided into Chinese medicine group and chemotherapy group,28 cases in each group.The Chinese medicine group received oral Xiangsha Liujunzi Decoction combined with R-CHOP regimen chemotherapy,and the chemotherapy group received R-CHOP regimen alone.21 days was a treatment cycle,and the clinical efficacy was evaluated every 4 cycles,30 healthy subjects were selected as the control group during the same period.Chinese and Western medical effects,symptoms of TCM,quality of life,T cell subsets,NK cell levels,changes of laboratory indicators and toxic side effects after chemotherapy were compared before and after treatment.Data processing analysis was performed using SPSS20.0 software package.Results:1.There was no significant difference in the general conditions of gender,age,pathological type,clinical stage,KPS score and TCM symptom score before treatment in the Chinese medicine group and the chemotherapy group,which were comparable.2.In the western medical efficacy evaluation,the total remission rate after treatment in the Chinese medicine group and the chemotherapy group was 92.86%and 89.29%,respectively,and there was no significant difference in the total remission rate between the two groups.3.The scores of TCM symptoms in the Chinese medicine group were significantly lower than those before treatment.There was no significant change in TCM symptom scores before and after treatment in the chemotherapy group.The TCM symptom scores in the Chinese medicine group were lower than those in the chemotherapy group after treatment(P<0.05).4.In the evaluation of TCM syndrome efficacy,the total effective rate of the Chinese medicine group and the chemotherapy group were 89.29%and 42.86%,respectively.The total effective rate of the Chinese medicine group was higher than that of the chemotherapy group,and the difference was statistically significant.5.The KPS score of the Chinese medicine group was significantly higher than that before treatment.There was no significant change in the KPS score before and after treatment in the chemotherapy group.The Chinese medicine group had a higher KPS score after treatment than the chemotherapy group(P<0.05).6.The levels of CD4~+,CD4~+/CD8~+and NK cells in the Chinese medicine group were significantly higher than those before treatment.The level of CD8~+was significantly lower than that before treatment,and there was no significant change in CD3~+level.The levels of CD4~+and CD4~+/CD8~+in the chemotherapy group were significantly higher than those before treatment.There were no significant changes in CD3~+,CD8~+,and NK cell levels.The control group had higher levels of CD3~+,CD4~+,CD4~+/CD8~+,NK cells and lower CD8~+levels than the Chinese medicine group and the chemotherapy group before treatment(P<0.05).There were no significant differences in the immune indexes between the Chinese medicine group and the chemotherapy group before treatment.The CD4~+,CD4~+/CD8~+,NK cells were higher and the CD8~+levels were lower in the Chinese medicine group than in the chemotherapy group after treatment(P<0.05).The levels of CD3~+,CD4~+and NK cells in the control group were higher than those in the Chinese medicine group after treatment(P<0.05).The levels of CD3~+,CD4~+,CD4~+/CD8~+and NK cells were higher in the control group than in the chemotherapy group after treatment,and the level of CD8~+was lower(P<0.05).7.The levels of WBC,NE,LY and PLT in the Chinese medicine group were significantly lower than those before treatment,and there was no significant change in Hb level.The levels of WBC,NE,LY,Hb and PLT in the chemotherapy group were significantly lower than those before treatment.There were no significant differences in blood routine parameters between the Chinese medicine group and the chemotherapy group before treatment.The level of Hb in the control group was higher than that in the Chinese medicine group and the chemotherapy group before treatment(P<0.05).There was no significant difference in the levels of WBC,NE,LY and PLT between the control group and the Chinese medicine group and the chemotherapy group before treatment.The levels of WBC,NE,LY and Hb in the Chinese medicine group were higher than those in the chemotherapy group after treatment(P<0.05),and there was no significant difference in PLT levels.The levels of WBC,NE,LY,Hb and PLT in the Chinese medicine group and the chemotherapy group after treatment were lower than those in the control group(P<0.05).8.The incidence of toxic side effects of the blood system in the Chinese medicine group and the chemotherapy group were 92.87%and 100%,respectively.The toxicity level of the blood system in the Chinese medicine group was lower than that in the chemotherapy group(P<0.05).9.The incidence of toxic side effects of the digestive system in the Chinese medicine group and the chemotherapy group were 42.86%and 78.57%,respectively.The incidence and degree of toxic side effects of the digestive system in the Chinese medicine group were lower than those in the chemotherapy group(P<0.05).Conclusion:1.Xiangsha Liujunzi Decoction has significant therapeutic effect on Spleen Stomach Qi Deficiency Syndrome in B-NHL patients.2.Xiangsha Liujunzi Decoction combined with R-CHOP treatment can improve the KPS score of patients with B-NHL and improve their quality of life.3.Xiangsha Liujunzi Decoction can enhance the cellular immune function of B-NHL patients.4.Xiangsha Liujunzi Decoction can reduce the degree of toxic side effects of blood system after R-CHOP chemotherapy in patients with B-NHL.5.Xiangsha Liujunzi Decoction can reduce the incidence and degree of toxic side effects of digestive system in patients with B-NHL after R-CHOP chemotherapy. |
