Polycystic ovary syndrome(PCOS)is one of the common disease of endocrine metabolic function in women of reproductive age.and also one of the major causes of anovulatory infertility.Anti-Mullerian hormone(AMH),also called Mullerian inhibiting substance(MIS),is a kind of important endocrine/paracrine hormone and a important indicator of ovarian reserve.Current studies have shown that anti-Müllerian hormone(AMH)is significantly related to follicular development,recruitment and contribute to the development of PCOS.Objective:This research will explore the relationship between AMH and PCOS,analze the difference between AMH in patients with polycystic ovary syndrome and normal women;The difference of AMH in different types of patients with PCOS;and evaluate relationship with various clinical parameters.Methods:According to Rotterdam criteria,88 patients diagnosed PCOS and 75 infertile patients during the same period in the reproductive clinic were divided into two groups.The reason of infertility were the male sperm abnormality or/and the woman with tubal factor.We measure the age,BMI,AMH,reproductive endocrine hormone,HOMA-IR,FPG,FINS and blood lipid level in these two groups.According to the BMI,ultrasonic,androgen levels,the clinic group,the PCOS patients divided into different groups.We compared the secretion of AMH level and evaluated its relationship with various clinical parameters.All the data were analyzed by SPSS20.0 statistical software.P<0.05 indicated the difference was statistically significant.Result:The level of AMH,LH,LH/FSH,PRL and T in PCOS group was significantly higher than those of normal women.(P<0.05)According to the ultrasound we divided PCOS patients into two groups(PCOM and non-PCOM): AMH in the PCOM group was significantly higher than in non-PCOM group.while the differences of LH,LH/FSH,T,P,SHBG,FINS,HOMA-IR level had no statistically significant.According to the diagnostic of hyperandrogenism we divided PCOS patients into two groups(HT and non-HT): in the HT group the levels of LH,FSH,LH/FSH,FT,SHBG,FPG,FINS,HOMA IR,FT3,triglycerides,blood cholesterol,LDL were significantly higher than in non-HT group.While the difference of AMH in these two groups had no statistically significant.According to the Rotterdam standard(European Society for human reproduction and embryology,2003)(ESHRE),PCOS patients can be divided into four groups: group 1 polycystic ovary change(PCO)+ oligo-ovulation or no ovulation(OA)+ hyperandrogenism(HA);Group 2(PCO+OA);Group 3(PCO + HA);Group 4(HA+OA).Although the relationship of serum AMH concentration was group 1 >group 2 >group 4 >group 3,。According to BMI we divided PCOS patients into obesity and non-obesity group: in the obesity group,T,FT,FT3,TG,TC,FINS and HOMA-IR were significantly higher than non-obesity group,while the AMH level is significantly lower than the non-obesity group.(P<0.05)Conclusion:The AMH level in the PCOS patients is significantly higher than that in the normal women,The AMH plays an important role in the mechanism of PCOS,This influence Maybe correlated with sex hormone disorder.The high level of AMH in patients with PCOS may be related to the PCOM status.The risk of insulin resistance in the HT patients is higher than that in non-HT patients with PCOS,There was no correlation between hyperandrogenism and AMH.According to clinic groups,the AMH level in group(PCO+OA+HA)is the highest.The AMH level in the obesity group is significantly lower than that in the non-obesity group.This influence maybe associated with glucose and lipid metabolic disorders.
