| Objective: To study the effects of methylprednisolone combined with tropisetron on postoperative nausea and vomiting(PONV)after gynecologic day surgery.Methods: Two hundred and eighty nine patients undergoing gynecologic day surgery were divided into two groups via random number table method(Group T N=143;Group T+M N=146).Observed the overall incidence of PONV in the two groups;PONV within6 h and 6-24 h was recorded.PONV in patients with different Apfel scores;The logistic regression analysis of PONV in both groups.VAS scores,Rammsay scores,and Visceral traction scores were also evaluated.Results: The incidence of urgent and delayed vomiting after the surgery in group T+M were significantly lower than those of group T.Patients whose Apfel score are 2-4 could benifit a lot from methylprednisolone and tropisetron in POV(P<0.05).Those Apfel score is 3 who have significantly lower incidence of PON(P<0.01).Risk factors for POV include postoperative opioid use,motion sickness or PONV history.There are no differences in VAS score,Ramssay Score,Visceral traction score between the two groups.Conclusion: Methylprednisolone combined with tropisetron is much better than tropisetron alone in reducing the incidence of PONV after gynecologic day surgery especially in urgent PONV and delayed POV.Those with higher Apfel scores could benifit a lot more from combined methyprednisolone and tropisetron. |
