ObjectiveTo understand the clinical nurses' perceptions and attitudes towards Nurse Practitioner,analyze the clinical nurses' expectations for the Nurse Practitioner,and explore the feasibility of scenario simulation combined with case analysis on the self-efficacy of becoming Nurse Practitioner?Methods1.Using the survey method combining quantitative and qualitative research,the quantitative research part adopts the self-made questionnaire and self-efficacy evaluation scale,and adopts the convenient sampling method to conduct the general situation of the nurses of 149 nurses in the tertiary hospital of Jiangsu Province,The survey of the nurses' cognition,career expectation,and self-efficacy of Nurse Practitioner.The qualitative research part selected 20 nurses by means of the purpose sampling method.The semi-structured interview method was used for qualitative interviews.After the informed consent was obtained,the recording was conducted.,transcription and collation analysis to understand the nurses' professional identity and expectations for nurse practitioners.2.Twenty-five specialist outpatient nurses from a certain city of Jiangsu Province were selected to conduct training and assessment on the knowledge of nurses in Nurse Practitioner.The situational simulation of the working nurses' work mode and case analysis were used to and explore the feasibility of scenario simulation combined with case analysis on the self-efficacy of becoming Nurse Practitioner?Results1.Quantitative research results:(1)Cognitive status survey: 63.09% of the nurses surveyed said that they did not know much about the Nurse Practitioner,and only11.41% of the nurses expressed a deep understanding of the Nurse Practitioner.The main channels for nurses in our province to obtain Nurse Practitioner are mainly concentrated in the literature,accounting for 53.02% of the total number.(2)Occupational expectation survey: 78.52% of nurses agree to develop the Nurse Practitioner in our province,and 88.59% of nurses intend to become Nurse Practitioner in practice,among which nursing graduates have the highest intention,accounting for 95% of the number.However,there are also concerns.36.24% of nurses believe that the primary factor affecting the development of the Nurse Practitioner is national policy orientation.44.30% of nurses are worried that their medical results will not be accepted by patients,and 38.26% of nurses are worried that the treatment technology is not enough.61.74% of the nurses believe that the nursing population of the Nurse Practitioner should be well-known experts in nursing,and 47.65% of the nurses think that the Nurse Practitioner should have graduate degree or above.(3)The investigation of the self-efficacy of nurses in different roles:the nurses of different roles showed different evaluation of the self-efficacy of becoming Nurse Practitioner,and the difference was statistically significant(P<0.05).Among them,the nursing professional master's degree(3.20±0.06 points)scored the highest,followed by the specialist nurses,respectively,cardiovascular nursing specialist nurses(3.07±0.07 points),wound ostomy nursing specialist nurses(3.05±0.09 points),intravenous infusion nursing specialist nurses(3.05±0.08 points),maternal and child care specialist nurses(3.03±0.06 points),nurses in the general ward had the lowest score(2.92±0.12 points).2.Qualitative research results show that: interviews with specialist outpatient nurses have refined five themes from positive intentions,including the development of the unanimous approval of Nurse Practitioner,the improvement of nursing status,the improvement of nursing convenience,the increased sense of nursing achievement,and the reasonable distribution of medical resources.In terms of negative intentions,four topics have been refined,including insufficient ability,conflict with doctors' opinions,and low patient acceptance.The lack of prescription rights for nurses.3.Through the intervention methods of scenario simulation and case analysis training for 20 nurses,the clinical nurses' self-efficacy scores on the role of nurse practitioners have been improved to some extent,and the difference is statistically significant(P<0.05).After the scenario simulated NP,the health consultation ability,clinical judgment ability,health education ability and independent prescription ability of self-assessment ability were improved,the difference was statistically significant(P<0.05),but the language expression ability was compared.The difference was not statistically significant(P>0.05).The quality of nurses' prescription quality after training was higher,and the difference was statistically significant(P<0.05).Conclusions1.Clinical nurses still have insufficient knowledge about the field of Nurse Practitioners.Nurses have low self-efficacy in Nurse Practitioners,but they show high professional expectations and willingness.They mainly show the phlegm of nurses' prescription rights and becoming Nurse Practitioners.Call on medical related departments to strengthen the promotion of Nurse Practitioner,nurse prescription rights,etc.Refer to foreign development experience,formulate relevant policies and regulations,responsibility system,strengthen postgraduate education and continuing education in China,and actively explore the road of Nurse Practitioner in China.2.The Nurse Practitioner in China are still in the exploratory stage.The health administrative department is the first to promote the Nurse Practitioner in the clinical departments with relatively independent outpatient qualifications such as wounds and ostomy clinics.3.Scenario simulation combined with case analysis can improve the self-efficacy of nurses in the process of simulating the Nurse Practitioner,ensure the correctness of nurses' prescriptions,and provide a good clinical training method for some post-care nurses or pre-service nurses with independent qualifications.At the same time,the training should focus on the cultivation of nurses' clinical judgment ability.
