Objective:To investigate the difference of bacterial culture of spinal tuberculosis between different focus tissues and different preoperative chemotherapy time during spinal tuberculosis operation.Methods:From November 2016 to February 2019,92 patients with spinal tubercu-losis underwent surgical treatment in our hospital,89 of them met the criteria and were divided into two groups: preoperative chemotherapy for 2 weeks group(41 cases,20 males and 21 females,aged 18-68 years,with an average age of 42.2 years);preoperative chemotherapy for 4 weeks group(48 cases,27 males and 21 females,aged 17-68 years,with an average age of 42.6 years).General data of patients were collected before operation in both groups,tuberculous granuloma,pus,necrotic bone and the boundary sclerosis bone were collected as culture materials during operation in all patients,after pretreatment,they were respectively cultured in modified L-J medium,the cultured positive patients were examined by Ziehl-Neelsen acid-fast staining microscopy and identified as Mycobacterium tuberculosis.Results:The positive results of tuberculous granuloma in 41 patients in the 2-week preoperative chemotherapy group were 16 cases,the positive rate was 39.02%;The positive results of pus were 9 cases and the the positive rate was 21.95%;The positive results of necrotic bone were 2 cases and the the positive rate was 4.88%;The positive results of the boundary sclerosis bone was 1 case and the the positive rate was 2.44%.In the 4-week preoperative chemotherapy group,the positive results were 27.08%,14.58%,2.08% and2.08%.Conclusion:In order to improve the positive rate,tuberculous granuloma should be selected as the preferred culture.Preoperative chemotherapy for spinal tuberculosis for 2 or4 weeks,the positive rate of intraoperative mycobacterium tuberculosis culture had no effect,preoperative chemotherapy was enough for 2 weeks.The boundary sclerosis bone should be thoroughly cleared during the operation of spinal tuberculosis so as to see normal bone tissue. |
