Objective: To investigate the influence of different pregnancy outcomes and related risk factors on EP by retrospective comparative study of 100 patients with EP and 200 patients with normal pregnancy and delivery at the same time,so as to promote gynecologists to understand the risk factors of EP more comprehensively,so as to reduce the incidence of EP.Methods: From June 2015 to June 2018,285 patients with EP and 7535 patients with normal pregnancy and delivery who were confirmed by surgical pathology or conservative treatment in the Department of Obstetrics and Gynecology of the First Hospital of Lanzhou University were selected as the study subjects.According to the inclusion and exclusion criteria,100 patients with EP were randomly selected as the case group,and 200 patients with normal pregnancy and delivery were randomly selected as the control group in a 1:2 ratio.The differences of delivery history,delivery times,delivery methods,abortion history,abortion times,conception methods,ectopic pregnancy history,age,smoking,marital status,education level,pelvic inflammatory disease history,appendectomy history,and tubal surgery history between the two groups were compared to explore the effects of different pregnancy outcomes and related risk factors on EP.Results: 1.The age of EP patients in the case group fluctuated between 19 and 45 years old,with an average age of 31.76±5.453 years.2.Different pregnancy outcomes,such as previous childbirth history,abortion history and ectopic pregnancy history,had significant differences between the two groups(P < 0.05),which were risk factors for EP.3.Pelvic inflammatory disease,fallopian tube surgery,appendix surgery,IVF-ET,unmarried,the differences between the two groups are statistically significant(P <0.05),which are all risk factors of EP.4.The difference of delivery methods between the two groups is not statistically significant(P > 0.05).Cesarean delivery is not a risk factor for EP.The differences of education level,age and smoking history between the two groups were not statistically significant(P > 0.05).Education level,age and smoking were not risk factors of EP.5.Multivariate analysis showed that pelvic inflammatory disease history,tubal surgery,appendectomy,ectopic pregnancy,IVF-ET,delivery,abortion and unmarried were independent risk factors of EP.6.From June 2015 to June 2018,the proportion of EP patients with previous cesarean delivery history among all EP patients showed an upward trend year by year.7.The total number of EP patients from June 2015 to May 2016 accounted for3.9% of the total number of patients with normal pregnancy and delivery in the same period.The total number of EP patients from June 2016 to May 2017 accounted for1.7% of the total number of patients with normal pregnancy and delivery in the same period.From June 2017 to June 2018,the total number of EP patients accounted for5.9% of the total number of patients with normal pregnancy and delivery in the same period.From June 2015 to June 2018,the overall incidence of EP showed an upward trend.Conclusion: 1.Birth history and abortion history are independent risk factors of EP;The risk of EP increases with the number of deliveries and abortions.2.Pelvic inflammation,tubal surgery,appendectomy,ectopic pregnancy,IVF-ET and unmarried are independent risk factors of EP.3.Compared with vaginal delivery,cesarean section did not increase the risk of EP;education level,age and smoking were not related to the risk of EP.4.For women of childbearing age,appropriate sex education should be given to strengthen the guidance of contraceptive measures,prevent unwanted pregnancy,reduce the number of abortions,recommend appropriate delivery,encourage vaginal trial delivery.
