| Objective:To analyze the etiology and clinical features of neonatal Infectious Pneunonia(NIP).Methods:A retrospective study of 272 children with neonatal respiratory tract diagnoses from the Department of Neonatology,Kunming Medical University Children’s Hospital from March 1,2017 to February 28,2018,including bacteriology,virology,and collection Clinical data of 228 common pathogens used to analyze the pathogenicity and clinical features of NIP,including:etiology,birth weight,birth pattern,gestational age,gender,age of onset,season of onset,pre-hospital feeding,respiratory infection Patient exposure history,maternal perinatal condition,initial symptoms,lung signs,white blood cell(WBC),hypersensitive C-reactive protein(hs-CRP),chest positive lateral radiograph,clinical comorbidities,length of hospital stay and prognosis,etc.,statistical analysis to guide clinical treatment.Results:1.There were 272 positive pathogens in the deep respiratory secretions of this group,179 cases(65.8%)with positive virus results,and 93 cases(34.2%)with positive bacteria.The positive rate of virus positive detection was higher than that of bacteria;Respiratory Syncytial Virus(RSV)ranked first(83.2%),51 cases(55.0%)of Gram-negative bacteria in bacterial pathogens,39 cases(41.8%)of Gram-positive bacteria,and Gram-negative bacteria were positive The detection rate is higher than Gram-positive bacteria;Gram-negative bacteria are ranked first in Escherichia coli and Klebsiella pneumoniae,and Gram-positive bacteria are ranked first in Staphylococcus aureus;2.RSV infection in this group High incidence in autumn and winter,the seasonal difference between bacteria and virus was statistically significant(P=0.000);3.The first symptoms of this group were cough,RSV infection,cough,snoring,nasal obstruction and spitting It was more common than bacterial infection(P<0.05),and fever was less common than bacterial infection(P<0.05).4.This group of data showed signs of lung:RSV infection,wet snoring and wheezing were more common than bacterial infection(P<0.05),bacterial infection in the lungs auscultation often does not smell early Obvious vocal(P<0.05);5.Data.assisted examination of this group:WBC count of bacterial infection,hs-CRP was significantly higher than RSV infection(P<0.05);chest lateral lobe:this group of cases were spotted And small-scale high-density shadow changes are common;bacterial infection in the lungs increased more than the RSV infection(P<0.05);6.This group of data NIP common associated disease is congenital heart disease,hyperbilirubinemia Symptoms and gastrointestinal dysfunction;7.The prognosis of NIP in this group is good,and there are no deaths during hospitalization.Conclusions:1.The positive detection rate of NIP virus in this group is higher than that of bacteria;the common virus is RSV(83.2%),the common bacteria are Staphylococcus aureus(38.5%),Escherichia coli(31.2%),Klebsiella pneumoniae(15.1%),the positive detection rate of Gram-negative bacteria was higher than that of Gram-positive bacteria;2.The RSV epidemic in this group is mainly in autumn and winter(October-January);3.The first symptoms of NIP in this group are mainly cough.The signs of snoring and wheezing and/or wet sputum are more considering RSV infection.Early fever,WBC count and hs-CRP should be alert to bacterial infection;4.The common clinical complications of NIP in this group are congenital heart disease,hyperbilirubinemia and gastrointestinal dysfunction;5.This group of data suggests that NIP has a good prognosis. |
