Objective: To observe the clinical efficacy of Jiawei Bazhen Decoction combined with Derma-670 red light irradiation in the treatment of alopecia areata with deficiency of both qi and blood,evaluate its effectiveness and safety,and preliminarily explore its mechanism and advantages.Methods: 70 patients with alopecia areata with deficiency of both qi and blood who met the inclusion criteria were divided into two groups: treatment group and control group,with 35 cases in each group.Treatment group: Jiawei Bazhen Decoction for oral administration combined with Derma-670 red light irradiation,control group: Compound glycyrrhizin tablets for oral administration combined with Derma-670 red light irradiation.The course of treatment lasted for 8 weeks,and the lesion area of patients was recorded by taking pictures at baseline before treatment and at the 4th and 8th weeks after treatment,and various observation indexes were improved.After treatment,the effectiveness and safety were evaluated respectively.During the treatment,pay attention to observe whether the patients have adverse reactions.After 8 weeks of treatment,telephone follow-up will be conducted to understand the recurrence of the patients.Result: 1.A total of 3 patients dropped out in this study,and the actual number of completed cases was 67(33 in the treatment group,34 in the control group).There was no significant difference in gender,age and course of disease between the two groups(P > 0.05),which was comparable.2.Comparison of clinical efficacy: according to the size change of hair loss area,its clinical efficacy was evaluated.After 4 weeks of treatment,the total effective rate was 69.70%(23/33)in the treatment group and 50.00%(17/34)in the control group.After rank sum test,there was no significant difference in the total effective rate(P > 0.05).After 8 weeks of treatment,the total effective rate was 93.94%(31/33)in the treatment group and 79.41%(27/34)in the control group.After the rank sum test,the difference in the total effective rate was statistically significant(P < 0.05).3.Comparison of TCM syndrome scores: after 4 and 8 weeks of treatment,the TCM syndrome scores of both groups decreased compared with before,and the comparison between before and after within the group was statistically significant(P < 0.05).There was a significant difference in TCM syndrome scores between the treatment group and the control group(P < 0.05).4.Comparison of Dermatology Quality of Life Index(DLQI): After 8 weeks of treatment,the DLQI scores of the two groups of patients decreased compared with before,and the comparison between before and after the group was statistically significant(P < 0.05).The difference of DLQI score between the treatment group and the control group was statistically significant(P<0.05).Conclusion: Modified Bazhen Decoction has definite clinical efficacy in treating alopecia areata with deficiency of both qi and blood.It is superior to compound glycyrrhizin tablets in improving hair loss area,TCM syndromes,skin disease quality of life index,and has no obvious adverse reactions.It is worth further exploring its mechanism of action and clinical promotion.
