Fuyuan Huoxue Decoction Combined With Low Molecular Weight Heparin Calcium In The Prevention Of Lower Extremity Deep Vein Thrombosis After Total Knee Replacement

Objective: To study the effect of Fuyuan Huoxue Decoction Combined with low molecular weight heparin calcium on coagulation parameters and blood cell parameters of patients after knee arthroplasty,to explore the safety and feasibility of Fuyuan Huoxue Decoction on the prevention of lower extremity deep vein thrombosis after knee arthroplasty,and to demonstrate the role of combined Chinese and Western Medicine in reducing perioperative risk factors of orthopedics The curative effect of traditional Chinese medicine can provide reference for anticoagulant therapy in perioperative period of total knee arthroplasty.Methods: According to the diagnosis criteria,60 patients with knee osteoarthritis who were admitted to the Department of Orthopaedics and traumatology of Ruikang Hospital Affiliated to Guangxi University of traditional Chinese medicine from December 2018 to December 2019 were randomly divided into two groups: treatment group and control group,30 cases in each group.Only western medicine low molecular weight heparin calcium was selected for anticoagulation treatment in the control group In thetreatment group,Fuyuan Huoxue Decoction Combined with low molecular weight heparin calcium was selected for anticoagulation treatment after operation.The two groups were treated with the same general treatment measures at present.The course of treatment was 7 days.The coagulation parameters(APTT,PT,D-? polymer),blood cell parameters(HGB,PLT),postoperative induced flow,VAS pain of the two groups were observed at different time points before operation,1,4 and 7 days after operation respectively Pain score,DVT incidence of lower extremity and other indicators were compared between the two groups at different time points to evaluate the effect of Fuyuan Huoxue Decoction Combined with low molecular weight heparin calcium.Results: 1.The time of APTT,PT in the treatment group was longer than that in the control group.There was no significant difference in APTT,PT between the two groups(P > 0.05).There was statistical significance in APTT,PT on the 7th day after operation(P < 0.05).There was no significant difference between the two groups(P > 0.05),and there was significant difference between the two groups(P < 0.05).2.The results showed that there was no significant difference between the two groups(P > 0.05)in hemoglobin detection results before operation,the first and the fourth day after operation,and the difference in hemoglobin detection results on the seventh day after operation was statistically significant(P <0.05).3.The average postoperative drainage volume of patients in the control group was about 367.20 ml,and that in the treatment group was about 345.37 ml,which was significantly lower than that in the control group.There was comparability between the two groups(P < 0.05).4.The average VAS score of the control group was 4.9 points,and that of the treatment group was 3.5 points,which was significantly lower than that of the control group.5.There were two cases in the control group and one case in the treatment group.There was no significant difference between the two groups(P > 0.05).Conclusion: after total knee arthroplasty,increasing Fuyuan Huoxue Decoction on the basis of low molecular weight heparin calcium can effectively prolong the coagulation time,reduce the concentration of d-?polymer,inhibit platelet adhesion,reduce the blood viscosity,give full play to the role of promoting blood circulation and removing blood stasis,improve the hypercoagulable state of blood and relieve the pain of patients To reduce the amount of bleeding and the incidence of DVT.Its safety and effectiveness are higher than that of using low molecular calcium alone,which is worth further clinical research and application.
