Objective To study the maternal and infant perinatal period outcomes of pregnant women with premature birth caused by hypertensive disorder complicating pregnancy(HDP).Method From January 1,2017 to December 31,2018,a total of 200 cases of singleton pregnancy with premature birth caused by HDP in Affiliated Hospital of Binzhou Medical University and Yantai Affiliated Hospital of Binzhou Medical University were selected as research subjects and were included into study group.The gestational age ranged from 28 weeks to 36+6 weeks.Meanwhile,another 200 cases of singleton pregnancy of spontaneous preterm and preterm premature rupture of membranes with premature birth with the same gestational weeks during the same gestational weeks from two same hospitals were selected as control group.The clinical data of all subjects were analyzed by retrospective method.The differences of general maternal characteristics,pregnancy and preterm infants outcomes,as well as birth weights,Z cores of birth weights of preterm infants in different gestational age periods and the incidence of common complications in neonates were statistically analyzed.Result 1、The delivery weeks and pre-pregnancy BMI of the pregnant women in the study group were higher than those in the control group(30.0 vs 29.0,29.8 vs 28.6),and there were significant differences between two groups.(P<0.05).2、The cesarean section rate in the study group was 90.0%,higher than that in the control group(28.0%),and the difference was statistically significant(2=158.909,P<0.05).3、The birth weight and the mean Z score of birth weight of preterm neonates in study group were lower than those of control group(2450.0 vs 2825.0,-0.0860 vs 0.8213),and the differences between two groups were statistically significant(P<0.001).4、The birth weight and the mean Z score of birth weight of preterm neonates with gestational age≥32-33+6 weeks and gestational age≥34-36+6 weeks in study group were lower than those in control group(1950 vs 2360,-0.273±0.056 vs 0.773±0.993;2600 vs 2960,0.097 vs 0.853),and all the differences between two groups were statistically significant(P<0.001).5、The proportion of 1-minute Apgar score≤7 scores after birth was higher than that of control group(15.0%vs 6.0%),and the difference between two groups was statistically significant(x2=8.619,P=0.003).The incidence of mild asphyxia,pneumonia,gastrointestinal disorders and the proportions of preterm infants transferred to the neonatology department in study group were higher than those of control group(15.0%vs 6.0%,13.0%vs 5.5%;24.0%vs 8.0%,8.0%vs 2.0%,,36.2%vs 20.0%),and the differences between two groups were statistically significant(P<0.05).6、The proportions of cesarean section,gestational age and preterm infants transferred to the neonatology department in the PE group were all higher than those in the non-PE group(97.4%vs 66.7%,35.2 vs 36.0,45.4%vs 14.6%),there were significant differences between two groups(P<0.05).Conclusions Avoidance of elder maternal age and regulation BIM before pregnancy are important methods to prevent the occurrence and development of HDP.Because the incidences of neonatal asphyxia and other neonatal complications are higher in HDP pregnant women,so treatment and nursing of preterm baby should be strengthened.Clinically,for HDP pregnant women with severe conditions,it is of great significance to choose an appropriate mode of delivery and timely termination of pregnancy after weighing the advantages and disadvantages according to the specific situation of mother and fetus.
