Objective: To explore the correlation between thyroid stimulating hormone(TSH)levels and metabolic syndrome(MS)and its components in patients with subclinical hypothyroidism(SCH)in Baohe District of Hefei city.Methods: According to the operation manual produced by the “(TIDE)” project team,using cluster and stratified sampling methods,2784 cases of permanent residents aged over 18 years(residence time ? 5)Year)as the object of investigation.According to the exclusion criteria,a total of 2628 patients were included in the analysis,including 1420 males and 1208 females.The investigator asked the subject and filled out a questionnaire to complete the measurement of the subject's height,weight,waist circumference,blood pressure,and other indicators such as blood glucose,blood lipid,and thyroid function.According to the subject's thyroid function,it is divided into SCH group and normal thyroid function group(control group);according to whether the subject has MS,it is divided into MS group and non-MS group;according to the distribution characteristics of TSCH levels in the SCH group,The SCH component was divided into three subgroups by the three-division method,which are group A: low TSH level(4.21-6.45)m IU / L;group B: median TSH level(6.46-8.52)m IU / L;C Group: High TSH level(8.53-15.00)m IU / L.The differences of clinical parameters and biochemical indexes between different groups were compared.Logistic regression analysis was used to explore the risk of MS and its components and the risk factors of MS in patients with different TSH levels in SCH patients.Results:(1)The detection rate of SCH in the survey population of this group was 19.29%(507/2628),the detection rate of MS was 29.45%(774/2628),and the median MUI was 208.5 ?g / L.This group of people is generally overweight,and the nutritional status of iodine is in excess.According to the stratified analysis based on gender,it can be found that in the male population,the detection rates of 2h PG,Hb A1 c,TG,TC,LDL-C,SBP,DBP,WC,MUI,BMI,and MS are higher than women,and the difference is statistically significant.(P <0.001);the HDL-C,TSH levels,and SCH detection rates were lower than those of women,and the differences were statistically significant(P <0.001).There were no significant differences in age,FPG,FT3,FT4,and TPOAb between the male and female populations(P> 0.05).(2)Compared with the control group with normal nail function,the detection rate of female proportion,age,2h PG,TG,TC,LDL-C,TSH,SBP,DBP,WC,BMI and MS in the SCH group were increased.The difference was statistically significant(P <0.001);while its HDL-C level was lower than that of the control group,the difference was statistically significant(P <0.001).There were no significant differences in FPG,Hb A1 c,FT3,FT4,TPOAb,and MUI between the two groups(P> 0.05).(3)There were differences in the detection rates of 2h PG,Hb A1 c,TG,TC,LDL-C,HDL-C,SBP,WC,BMI,and MS between the three groups of group A,group B,and group C,with statistical significance(P <0.01);the levels of LDL-C and HDL-C in group C were significantly higher than those in group A,while the levels of 2h PG,Hb A1 c,TG,TC,SBP,WC,BMI,and MS were significantly higher than those in group A The difference between group B and group B was statistically significant(P <0.05).In addition,the levels of 2h PG and SBP in group B were significantly higher than those in group A,and the difference was statistically significant(P <0.05).(4)Compared with the control group,in patients with SCH,the risk of central obesity,hypertriglyceridemia,low HDL-Cemia,and MS was significantly increased in group C,which increased by 1.292 times(95% CI): 1.028-1.622,P <0.001),2.205 times(95% CI: 1.703-2.880,P <0.001),1.425 times(95% CI: 0.983-1.793,P <0.001),and 1.722 times(95% CI: 1.702)-2.885,P <0.001).After adjusting for age and gender,compared with the control group,the risk of hypertriglyceridemia in the group B increased by 1.182 times(95% CI: 1.072-1.010,P <0.05);the central obesity,high The risk of TGemia,elevated blood pressure,and MS increased by 1.280 times(95% CI: 1.018-1.620,P <0.001),2.150 times(95% CI: 1.625-2.789,P <0.001),and 1.158 times(95 % CI: 1.107 ~ 1.914,P <0.001),1.912 times(95% CI: 1.700-2.992,P <0.001).(5)Application of logistic regression analysis found that TSH,TG,and LDL-C were independent risk factors affecting MS in this group,and the OR values were 1.850(95% CI: 1.625-2.002)and 1.394(95% CI: 1.246-1.560),1.493(95% CI: 1.118-1.994).Conclusion: The MS component of SCH patients in Baohe District of Hefei City is more obvious,and the proportion of MS is higher.TSH is an independent risk factor for the onset of MS.As the level of TSH increases,the risk of MS increases.Strengthening the screening of SCH,the evaluation of MS and its components in SCH patients,and timely intervention treatment will help prevent and treat MS and cardiovascular diseases in the region.
