| Objectives To observe the clinical effect and the level of NO、hs-CRP change of Shentongzhuyu decoction combined with acupoint application in the treatment of unstable angina pectoris whose symptom belongs to intermingled wind and blood stasis,explored its possible mechanism,to provide new ideas and methods for TCM treatment of unstable angina pectoris.Methods The patients were selected from September 2018 to September 2019 in the inpatient ward of Shijiazhuang Municipal hospital of Traditional Chinese Medicine collected a total of 93 cases are in line with inclusion criteria.According to the actual situation of clinical patients with complications,following the principles of randomization and balance,the patients were divided into control group,treatment group 1 and treatment group 2 by hierarchical block randomization.During the treatment period,the three groups should be given general non drug treatment,such as bed rest(1-3 days),ECG monitoring,finger pulse oxygen detection,oxygen inhalation,smoking cessation and alcohol restriction;The control group was given basic medication of Western medicine,The treatment group 1 was given with Shentong Zhuyu Decoction combined with basic medication of Western medicine,while the treatment group 2 was given basic medication of Western medicine +acupoint application+Shentong Zhuyu Decoction.The selected points are Neiguan,Tanzhong and Xinshu,with a total of 7 patches,once every other day,one patch per point for 6 hours,and the treatment period is 14 days.The levels of nitric oxide(NO),hypersensitive C-reactive protein(hs-CRP),angina pectoris attack score(pain degree,attack frequency,duration),TCM symptom score,TCM syndrome curative effect,angina pectoris curative effect,electrocardiogram change,Seattle Angina Pectoris Scale score before and after treatment were statistically analyzed by SPSS22.0 to determine the curative effect of the three groups.Results After 14 days of treatment,ninetuy patients successfully completed the experiment,and three patients fell off.The results were in line with the expected judgment:1.In terms of hs-CRP level,the volue of hs-CRP level of the three groups after treatment was stronger than that before treatment,and the difference was statistically significant(P<0.05).Pairwise comparison after treatment,there was evident difference between the control group and other two groups(P<0.05);there was significant difference between treatment group 1 and treatment group 2(P<0.05).2.In terms of NO level,the volue of No level in the three groups after treatment was improved,the difference was statistically significant(P<0.05).Pairwise comparison after treatment,there was comparable difference between the control group and other two groups(P<0.05);there was significant difference between treatment group 1 and treatment group 2(P<0.05).3.From score of angina attack,the effect of the three groups after treatment improved compare with before treatment,and the difference was statistically significant(P<0.05).Pairwise comparison after treatment,there has statistical significance between the control group and other two groups(P<0.05);there was distinct difference between treatment group 1 and treatment group 2(P<0.05).4.In the aspect of TCM symptom score,the effect of the three groups after treatment stronger than that before treatment,and the difference was statistically significant(P<0.05).Pairwise comparison after treatment,there was distinct difference between5.the con-trol group and other two groups(P<0.05);there was significant difference between treatment group 1 and treatment group 2(P<0.05).6.In the treatment of angina pectoris,the data of the three groups were compared by rank sum test,with statistical significance(P<0.05).Pairwise comparison through SNK-q test,there was evident difference between the control group and other two groups(P<0.05);there was significant difference between treatment group 1 and treatment group 2(P<0.05).treatment group 2 had the best curative effect on the improvement of angina pectoris symptoms.7.As for the curative effect of TCM symptoms,the data of the three groups were compared by rank sum test,with statistical significance(P<0.05).Pairwise comparison,there was obvious difference between the control group and other two groups(P<0.05);there was significant difference between treatment group 1 and treatment group 2(P<0.05).In addition,the curative effect of TCM symptoms of treatment group 2 improved most significantly.8.In terms of electrocardiogram curative effect,the data of the three groups were compared by rank sum test,with statistical significance(P<0.05).Pairwise comparison,the control group was compared with the other two groups(P<0.05),there was significant difference between treatment group 1 and treatment group 2(P<0.05);The treatment two groups were compared(P<0.05),showed that electrocardiogram in treatment group 2 improved most significantly.9.In terms of score of Seattle Angina scale,the treatment effect of the three groups in the five dimensions of PL,AS,AF,TS,DS after treatment was stronger than that before treatment,and the difference was statistically comparability(P<0.05).Pairwise comparison after treatment,there was significant difference between treatment group 1 and treatment group 2 in the dimensions of PL,AS,AF and TS(P<0.05),the effect of treatment group 2 was the best;there was no significant change in the dimension of DS(P>0.05)Conclusions The preliminary results showed that the combination of Shentongzhuyu Decoction and acupoint application could significantly improve the curative effect of TCM symptoms,angina pectoris and NO level of patients with unstable angina pectoris,reduce the level of hs-CRP,ameliorate the ECG,the degree of physical activity limitation,the stable state of angina pectoris,the attack of angina pectoris,and improve the patients’ satisfaction of disease treatment,no damage of liver and kidney function.It can be concluded that the clinical effect of Shentongzhuyu decoction combined with acupoint application is significant in patients with unstable angina pectoris.The related mechanism may be to inhibit inflammation,improve endothelial cell function,relieve coronary spasm,increase coronary blood supply,and delay the
