| Objective: Tinea manus and tinea pedis is a highly infectious disease caused by superficial fungi,which seriously affects the health and quality of life of patients because of its high incidence rate and easily relapse.Scaly keratotic tinea manus and tinea pedis is the most common and difficult type of tinea manus and tinea pedis.In this study,we compared the clinical efficacy of fire needle therapy and positive antifungal drugs in the treatment of scaly keratotic tinea manus and tinea pedis,so as to provide basis for the clinical application of fire needle in the treatment of scaly keratosis manus and pedis,and promote the popularization and application of fire needle therapy in the treatment of tinea manus and pedis.At the same time,through the research to verify the efficacy of fire needle to remove fungi,in order to enrich the application of fire needle therapy in the treatment of fungal dermatosis.In this study,we compared the clinical efficacy of fire needle therapy and conventional external antifungal drugs in the treatment of scaly keratotic tinea manus and tinea pedis.At the same time,the application of fire needle in fungal dermatosis should be promoted.Methods: A randomized controlled trial was used in this study.From November 2018 to November 2019,a total of 70 patients who were definitely diagnosed as scaly keratotic tinea manus and tinea pedis in Sichuan Second Hospital of traditional Chinese Medicine were selected and randomly assigned to thefire needle group or the NHKC group,35 cases in each group.The treatment group was treated with fire acupuncture once a week for 4 weeks,and the control group was treated with NHKC cream twice a day for 4 weeks.Before treatment,after treatment and after follow-up,the eight clinical symptoms of the two groups were scored according to the four-level scoring method,including the area of skin lesions,erythema,papule,desquamation,keratosis,rhagadia,pruritus and pain,and the specimens were scraped before and after treatment and after follow-up for fungal microscopy.According to the changes of symptom and sign scores,fungal clearance and recurrence of the disease after 1 month followup,the clinical efficacy was determined.Results: 1.Comprehensive effect:The total effective rate of fire needle group was 82.4%,and that of NHKC group was 57.1%,the difference was statistically significant(P<0.05).2.Total score of symptoms and signs(TSS):After treatment,TSS in the fire needle group was 3.41 ± 2.27 and TSS in the NHKC acupuncture group 4.74 ± 2.93(P<0.05),and there was significant difference between the two groups.After follow-up,the TSS of fire needle group was 3.32±2.32 and that of NHKC group was 4.83±2.99(P<0.05),there was significant difference between the two groups.3.Improvement of single symptoms:There was no significant difference between the two groups(P > 0.05)in the improvement of erythema,papule,esquamation,hagadia,pruritus and pain,and the improvement of keratosis and lesion area was better than that of NHKC group(P<0.05).4.Fungal clearance rate:At the end of the treatment,the fungal clearance rate of the fire needle group was 70.6%,and that of the NHKC group was 91.4%(P<0.05).That is to say,the efficacy of NHKC cream was better than that of the fire needle group.After 1 month of follow-up,the fungal clearance rate of the fire needle group was 67.6%,and that of the NHKC group was 77.1%(P>0.05).There was no significant difference in the longterm fungal clearance rate between the two groups.5.Recurrence rate:The patients who were basically cured were followed up for one month.There was no signifycant difference(P>0.05).Conclusion: The clinical effect of fire needle therapy is definite,it can improve the clinical symptoms of patients,especially in improving the keratosis of skin and the area of skin lesions.The fire needle therapy has a certain effect of removing fungi,which can be used in the treatment of superficial dermatomycosis.And the fire needle therapy has the advantages of post acupuncture effect after the treatment,high safety and low cost,which is worthy of clinical application. |
