Objectives To investigate the expression levels of adiponectin,apelin and chemerinin serum of patients with diarrhoeal irritable bowel syndrome(IBS with diarrhea,IBS-D).The relationship between adiponectin,apelin,chemerin and the severity of gastrointestinal symptoms and anxiety and depression in patients with IBS was analyzed.To investigate the possible role of adiponectin,apelin and chemerin in the pathogenesis of IBS.It provides a new theoretical basis for the study of the pathogenesis of IBS-D and clinical diagnosis and treatment.Methods IBS-D patients were included in the outpatient clinic of digestive department of Hebei Province People's Hospital from March 2019 to December 2019 and met the Roman ?diagnosis standard.During the same period,15 healthy physical examinees were randomly selected from the physical examination center of Hebei Province People's Hospital as the healthy control group(group N).The state of anxiety and depression of IBS-D patients was assessed by self-rating anxiety scale(SAS)and self-rating depression scale(SAS).According to the results of SAS and SDS,IBS-D patients were further divided into the following two groups: the group with anxiety and/or depression(M group,n=20)and the group without anxiety and/or depression(B group,n=15).The general clinical data of all subjects were recorded and fasting serum samples were collected in the morning.The expression levels of adiponectin apelin,chemerin in the fasting serum of the three groups were detected by enzyme linked immunosorbent assay(ELISA).The IBS symptom scale(IBS-SSS)was used to assess the severity of IBS-D patients' symptoms.The expression levels of adiponectin,apelin and chemerin in serum between the M,B and N groups were examined by one-way anova.The expression levels of adiponectin,apelin and chemerin in serum between the IBS-D group and the healthy control group were analyzed by independent sample t test,as well as the scores of SAS,SDS and IBS-SSS.Pearson correlation analysis was used to evaluate adiponectin and apelin in the IBS-D group.The correlation between serum expression level of chemerin and SAS score,SDS score and IBS-SSS score.Results 1 Basic data analysis of the subjects: gender,age and BMI were statistically analyzed among groups M,B and N,and the differences were not statistically significant(P >,0.05).2 Evaluation of serum adiponectin,apelin and chemerin expression levels: the expression level of adiponectin in IBS-D group was significantly lower than that in N group(4785.412±1010.678 vs.6226.308±704.838,P < 0.05).Among them,the expression level of adiponectin in group M was lower than that in group B(4224.1±880.748 vs.5537.827±605.361,P < 0.05).The expression level of apelin in IBS-D group was significantly lower than that in N group(954.0629±337.928 vs.2031.249±695.835,P < 0.05).Among them,the expression level of apelin in group M was significantly lower than that in group B(779.614±189.151 vs.1186.661±356.838,P < 0.05).The expression level of chemerin in IBS-D group was significantly higher than that in N group(7.811±1.097 vs.4.968±1.790,P < 0.05).Among them,the expression level of chemerin in group M was higher than that in group B(8.222±0.890 vs.7.264±1.135,P < 0.05).3.State assessment of anxiety and depression: SAS score and SDS score of IBS-D patients were significantly higher than that of N group(50.05±10.35 vs.37.27±6.78,P < 0.05)(45.23±8.41 vs.37.87±9.18,P < 0.05).4 Evaluation of symptom severity: IBS-SSS score of IBS-D group was significantly higher than that of N group(223.50±64.32 vs.54.67±11.25,P < 0.05),among which,IBS-SSS score of M group was higher than that of B group(240.50±86.60 vs.54.67±11.25,P < 0.05).5 Correlation analysis of serum adiponectin,apelin and chemerin levels in IBS-D patients with SAS score and SDS score: serum adiponectin expression levels were negatively correlated with SAS score(r =-0.583,P < 0.05)and SDS score(r =-0.550,P < 0.05).Serum apelin expression level was negatively correlated with SAS score(r =-0.633,P < 0.05)and SDS score(r =-0.504,P < 0.05).Serum chemerin expression level was positively correlated with SAS score(r = 0.633,P < 0.05)and SDS score(r = 0.580,P < 0.05).6 Correlation analysis between serum adiponectin,apelin and chemerin expression levels and IBS-SSS score in IBS-D patients: serum adiponectin expression levels were negatively correlated with IBS-SSS score(r =-0.337,P < 0.05).The expression level of serum apelin was negatively correlated with IBS-SSS score(r =-0.375,P < 0.05).Serum chemerin expression level was positively correlated with IBS-SSS score(r = 0.652,P < 0.05).Conclusions 1 Reduced adiponectin and apelin levels and increased chemerin levels in patients with IBS-D were correlated with the severity of symptoms.Adiponectin,apelin and chemerin may be involved in the pathogenesis of IBS-D.2 Adiponectin,apelin and chemerin may be involved in the regulation of anxiety and depression in patients with IBS-D.Figure0;Table 6;Reference 143...
