| Background: According to the World Health Organization,breast cancer is the most common cancer among women in the word,and the incidence has been increasing.In China,breast cancer is also one of the most common malignant tumors,which seriously threatens women’s physical and mental health.Not only has the incidence rate increased much faster than some developed countries,but the incidence has shown a younger trend in recent years.Since the 1990 s,with the popularization of early screening(such as breast color doppler ultrasound and the mammography)and the improvement of clinical treatment technology,the mortality rate of breast cancer has been decreasing and the survival rate has been increasing,leading to an increasing number of breast cancer survivors.Therefore,it is very important to help these people to adapt to the long-term living condition with cancer,return to society better,reduce the burden of family and improve their quality of life(QoL).However,the current research on the quality of life of breast cancer patients are mostly limited to the patients within 5 years after surgery,few studies are aimed at patients whose survival time is longer than 5 years after surgery.Therefore,this study investigated the quality of life of long-term survivors of breast cancer,as well as the related factors affecting the quality of life.To study this problem can provide scientific basis for clinical or community medical staff to develop personalized follow-up management measures for long-term survival of breast cancer,so as to improve the physical and mental health of patients.Objective: To investigate the quality of life,psychological status(anxiety and depression)and physical symptoms(cancer fatigue)of long-term breast cancer survivors in China,and to explore the relationship between the quality of life,to provide scientific basis for follow-up management measures to improve the quality of life.Methods: A multicenter cross-sectional study was used in this study.The patients who survived for 5 years or more were selected from the breast surgery / oncology department of the third-grade class-A hospital in Shiyan City.The self-designed demographic questionnaire,the Chinese version of Zung Self-rating Anxiety Scale(SAS),Zung Self-rating Depression Scale(SDS),Revised Piper Fatigue Scale(RPFS),the Functional Assessment of Cancer Therapy-Breast(FACT-B)were used to collect the data of demographic,anxiety,depression,fatigue and quality of life.Statistical description(percentile,mean,standard deviation,etc.)were used to describe the sample.Pearson correlation analysis the correlation between general demographic data,anxiety,depression,fatigue with the quality of life of the,Multivariate linear regression(stepwise)analysis the significant factors affecting quality of life.Results: 122 female long-term breast cancer survivors who survived 5-10 years after operation participated in this study.The average age was 51.39 years(± 6.56 years),the average survival time was 6.36 years(± 1.69 years);the anxiety score was 35.89(± 7.45),of which 8.2% had mild anxiety;the depression score was 35.85(± 7.85),of which 4.1% had mild depression and 0.8% were moderate depression;the fatigue score was 3.04(± 2.55),a total of 67.2% were at different levels of fatigue,of which 14.8%(18)were severe fatigue;FACT-B was 53-136 points(111.51 ± 14.27).Pearson correlation showed a positive correlation between family monthly income and quality of life(r = 0.319,P <0.01);anxiety(r =-0.720,P <0.01),depression(r =-0.785,P <0.01),fatigue(r =-0.610,P <0.01)has a negative correlation with quality of life;multiple linear regression shows that anxiety(β =-0.272,P <0.01)and depression(β =-0.531,P <0.01)are the two most predictions with the quality of life.Conclusion: The quality of life of long-term breast cancer survivors in China is good,but there are still different levels of anxiety,depression,cancer fatigue and other health problems in this population.Anxiety and depression seriously affect their quality of life.Therefore,medical staff should pay attention to the quality of life of long-term survivors of breast cancer,especially their psychological changes.Medical staff should provide psychological consultation to improve the quality of life of long-term breast cancer survivors through regular follow-up. |
