| Objective: To investigate the relationship between plasma N-terminal pro brain natriuretic peptide(NT-proBNP),Cystatin C(Cys-C)levels and the occurrence of acute ischemic stroke(AIS),infarct size and the degree of neurological deficit, and to further analyze their related influencing factors and their relationship with each other. Methods: From January 2018 to June 2019,89 first-acute ischemic stroke patients admitted to the Affiliated Hospital of Hebei Engineering University were selected as the case group, and 88 healthy people were selected as the control group. The plasma NT-proBNP and Cys-C levels were detected and compared between the case group and the control group.According to the infarct size, the case group was divided into largearea infarction,medium-area infarction and small-area infarction.According to the National Institute of Health stroke scale(NIHSS),the case group was divided into the severe group and the mild group. The plasma NT-proBNP and Cys-C levels were compared between different infarct size groups and different NIHSS score groups.Pearson or Spearman correlation was used to analyze the correlation between plasma NT-proBNP,Cys-C levels and other clinical indicators.Logistic regression was used to analyze the relationship between plasma NT-proBNP,Cys-C levels and the occurrence of AIS.Results:1.The factors of smoking history, hypertension history, diabetes history, age, total cholesterol, low density lipoprotein, lipoprotein(a)and plasma NT-proBNP,Cys-C levels in the case group were significantly higher than those in the control group(P<0.05).2.The plasma NT-proBNP,Cys-C levels in the large-area infarction group were significantly higher than those in the medium-area infarction group(P <0.05),and plasma NT-proBNP,Cys-C levels in the medium-area infarction group were significantly higher than those in the small-area infarction group(P <0.05).3.The plasma NT-proBNP levels in the severe group was significantly higher than that in the mild group(P <0.05).However,there was no significant difference in plasma Cys-C levels between the severe group and the mild group(P <0.05).4.The plasma NT-proBNP,Cys-C levels in patients with AIS were positively correlated with age and creatinine(P <0.05),and the plasma NT-proBNP levels were positively correlated with Cys-C levels and NIHSS score(P <0.05).5.Multivariate logistic regression analysis showed that after adjusting for confounding factors, the plasma NT-proBNP,Cys-C odds ratio(OR),and 95%confidence interval(CI)respectively were 1.022(1.011~1.033)and 6.60(1.279~34.048).Conclusions:1.The increase of plasma NT-proBNP,Cys-C levels is independently related to the occurrence of acute ischemic stroke.2.The plasma NT-proBNP,Cys-C levels in patients with AIS can reflect the severity of the disease to a certain extent.3.The plasma NT-proBNP,Cys-C levels in patients with AIS were positively correlated with age and creatinine. The influence of these factors should be considered when evaluating the levels of NT-proBNP and Cys-C in order to evaluate the disease objectively. |
