Effect Of Shenfu Injection On Blood Coagulation And CRP In Patients With Multiple Trauma And Shock

Objective:To study the effect of early low-dose Shenfu injection on coagulation,inflammatory reaction and prognosis in patients with multiple injury and shock.Methods:A prospective study was conducted in 50 patients with multiple injuries and shock in the Emergency intensive-care unit of the Second Affiliated Hospital of Nanchang University from November 2017,All patients were randomly divided into control group and experimental group with 25 cases each,The control group was given general antishock therapy,In the experimental group,Senfu injection 50 ml plus saline 100 ml solution was added to the control group and give it intravenously once a day.Both groups were treated for at least five days.Then compare the epidemiological characteristics and the use of blood products after five days of hospitalization between the two groups.In addition the Prothrombin time(PT)、Partial activated Thromboplastin time(APTT)、Thrombin time(TT)、D-DIMER(D-D)、Fibrinogen(Fib)、Platelet count(PLT)and C-reactive protein(CRP)after three and five days of treatment in both groups.Finally,the experimental data of the two groups were analyzed statistically.Results:The two groups of patients were mainly young and middle-aged male patients.The main injury mode was traffic injury,and the main injury sites were head,chest and abdomen,There was no significant difference between the two groups(P>0.05).and there was no significant difference between the two groups in trauma severity score and the amount of blood products used in the first 5 days(P>0.05).At the time of admission,D-D and CRP were significantly higher than normal,PLT count and Fib concentration were lower than normal,The PT and TT values were longer than the normal values,and there was no significant difference between two groups(P>0.05).PT,TT and D-dimer in the experimental group were significantly decreased after 3 days of treatment,and platelet count and fibrinogen concentration were also increased,compared with the control group,the differences were statistically significant(P<0.05).The levels of CRP in both groups were higher,but the increase in the experimental group was not significant,the differences between two groups were statistically significant(P<0.05).the APTT decreased slightly in both groups,But there was no significant difference between the two groups(P>0.05).After five days of treatment,the PT,TT,d-dimer and CRP in the two groups were decreased,but the decrease was significant in the experimental group,Compared with the control group,there was significant difference(P<0.05).The PLT count of the two groups were higher than before,but the experimental group was significantly higher,the difference between the two groups was statistically significant.Conclusion:The early use of Shenfu injection can have a benign regulatory effect on the patient’s coagulation system.In addition,Shenfu injection can also reduce the systemic inflammatory response in patients with multiple injuries.
