Objectives(1)To investigate the fear of cancer recurrence in patients with cervical cancer after surgery and explore its influencing factors.(2)To explore the intervention effect of cognitive behavioral therapy on the fear of cancer recurrence and quality of life in patients with cervical cancer after surgery,and to enrich the theoretical framework for interventional research on cancer patients fear of cancer recurrence.MethodsPart 1:214 patients with cervical cancer who were treated in a tertiary oncology specialist hospital in Tianjin from May to August 2019 were investigated by convenience sampling method.The general information questionnaire,Chinese version of Fear of Cancer Recurrence Inventory(FCRI),Medical Coping Modes Questionnaire(MCMQ)and Hospital Anxiety and Depression Scale(HADS)were used as research tools to conduct cross-sectional survey.SPSS25.0 statistical software was used to investigate the current status and influencing factors of fear of cancer recurrence in patients with cervical cancer after surgery through univariate,correlation analysis and multiple linear regression analysis.Part 2:Using convenient sampling method,64 patients with cervical cancer who were hospitalized in a third grade a cancer hospital in Tianjin from August to November 2019 were selected as the research objects.The coin toss method was used to randomly divide the two floors into an intervention group and a control group.The patients in the control group received routine nursing.The intervention group was given cognitive behavioral intervention for 6 times,40-60 minutes each time.The level of fear of cancer recurrence and the quality of life of cancer patients were measured before and after the intervention in both groups.The independent sample t test,non-parametric rank sum test,?~2test,and Fisher's exact probability method in SPSS 25.0 statistical software were used to analyze the differences between the main scale scores of the two groups of patients before and after the intervention and the differences between the groups.And to explore the effect of cognitive behavioral intervention on cervical cancer patients.ResultsPart 1:(1)The fear of cancer recurrence inventory score of in patients with cervical cancer after surgery was(98.34±20.07),the score of dimension were:the trigger factor was(18.60±3.26),the severity was(20.21±5.47),psychological distress was(8.72±3.22),coping strategy was(22.36±6.22),functioning impairments was(14.00±3.48),insight was(6.89±1.84),reassurance was(7.67±1.75).(2)The scores of medical coping modes questionnaire of postoperative patients with cervical cancer were coping(20.31±3.42);coping avoidance(18.31±3.26);yielding coping(13.00±2.66);Anxiety scores of patients after cervical cancer operation was(7.57±3.55)and depression score was(8.67±2.60).(3)One-way analysis of variance showed that there were significant differences in age,education level,family monthly income,occupational status,main caregivers,marital status,medical payment,recurrence,comorbidity and family cancer history among cervical cancer patients(P<0.05).Pearson correlation analysis showed that patients with cervical cancer after surgery The total score of fear cancer recurrence level and each dimension were negatively correlated with the coping style of medical coping styles(P<0.05)and positively correlated with the coping style of yielding(P<0.05),the total score of the level of fear of cancer recurrence and its dimensions in patients with cervical cancer after surgery were positively correlated with anxiety and depression scores(P<0.05).(4)The multiple linear regression analysis results showed that age,education level,recurrence,coping,yielding response,and anxiety and depression entered the regression equation(P<0.05),which are important factors affecting the fear of cancer recurrence in cervical cancer patients.Part 2:(1)Comparison of baseline levels of patients in the two groups:There was no statistical difference in the baseline data,the level of fear of cancer recurrence,and the quality of life between the two groups(P>0.05);(2)Comparison of fear of cancer recurrence between the two groups of patients:comparison within the group showed that the score of fear of cancer recurrence of patients in the intervention group before and after the intervention was lower than the baseline score,and the difference was statistically significant(P<0.05),while the control group had no statistics difference(P>0.05);after intervention,the fear of cancer recurrence inventory scores in the intervention group was lower than that of the control group,and the difference was statistically significant(P<0.05);(3)Comparison of the quality of life of the two groups of patients:comparison of the intervention group before and after the intervention in the group Patients'overall quality of life and functional domain dimensions scores significantly improved,and the differences were statistically significant(P<0.05);the scores of physical function and role function in the control group were statistically significant(P<0.05),while the scores of other functional dimensions and overall quality of life were not statistically significant(P>0.05);The results of the intervention study showed that there were significant differences in the overall quality of life score,functional domain dimension score and insomnia score between the two groups(P<0.05);there was no significant difference in other symptom domain scores(P>0.05).Conclusions(1)Patients with cervical cancer have a higher level of fear of cancer recurrence,which is affected by factors such as age,education level,recurrence,coping style,yielding dimension,facing dimension,and anxiety and depression.(2)Cognitive-behavioral interventions have a positive effect on improving the relapse fear of cancer cancer recurrence and quality of life of patients with cervical cancer,and the effect is better than that of the conventional care group.Therefore,cognitive-behavioral interventions are suitable for alleviating the fear of cancer recurrence in patients with cervical cancer and to promote application in cancer patients.
