Background: Chronic low back pain(CLBP)is a common symptom of waist.CLBP leads to a large economic burden and many disabled problem,it has became a hot research topic for rehabilitation medical workers.Previous study demonstrated that there is a close association between neuromuscular function and CLBP.Recently,whole body vibration(WBV)has emerged as a new type of neuromuscular training method,and it has become increasingly popular in sports and clinical sectors.WBV training can improve the proprioception and relieve pain in patients with chronic low back pain,but Long-term WBV could lead to low back pain,and high frequency of WBV training could cause muscle fatigue.Different parameters of WBV has different effects on patients with CLBP.There is still have any studies focused on it.Therefore,we aim to explore the influence of vibration stimulation on chronic low back pain patients lumbo-abdominal major muscles under different vibration stimulation conditions,and to determine the best vibration frequency and location by surface electromyogram(s EMG)analysis.Methods: s EMG signals from the lumbo-abdominal major muscles of 21 chronic low back pain patients were measured with the Myo Research XP Master Software Version 1.07.17(Noraxon Inc.,Scottsdale,USA).Each subjects performed 3 exercises on 0hz/5hz/10hz/15 hz a vibration platform: single bridge,plank,side stay and the“V support”.The surface electromyography(EMG)signals were collected from four muscles: the erector muscle,the abdominal oblique muscle and the rectus abdominis muscle.We analyze the data from three aspects,first: the effects of differentvibration frequencies on lumbo-abdominal muscles in CLBP patients,second:theeffects of different vibration positions on lumbo-abdominal muscles in CLBP patients,third,the interaction effect between frequencies and positions.The data were analyzed by repeated measure ANOVA.Results:(1)the surface electromyography of multifidus muscles: frequency and positions have interaction effects(P=0.01<0.05).Under plank,there were significant differences between different frequency(P<0.05).At the frequency of 15 Hz,the increase value of surface electromyography is maximum.Under 10 hz and 15 Hz,the multifidus muscles showed significant difference between different exercises(P<0.05).At the position of plank,the increase value of surface electromyography is maximum.(2)the surface electromyography of erector spinae muscles: There was no interaction effects between frequency and action(P=0.254 > 0.05);There was significant difference between different frequencies in the single bridge movement(p<0.05),with the frequency of 15 Hz,the surface electromyography of the erector spinae muscles increased most.(3)the surface electromyography of the external oblique muscles: there is no interactive effects between frequencies and positions(P=0.466 > 0.05);Under the position of single bridge,plank support and lateral support,there was significant difference between different frequencies(p<0.05),with the frequency of 15 hz,the surface electromyography of the external oblique muscles increased most.At the frequency of 15 Hz,there was significant difference between the different positions(p<0.05).In the single bridge,the increase value of the surface electromyography muscle was the largest.(3)The surface electromyography of rectus abdominis muscle: There was no interactive effects between frequencies and positions(P=0.676 > 0.05);There was significant difference between different frequencies in single bridge and side stay(p<0.05).At the frequency of 15 Hz,the increase value of the surface electromyography muscle was the largest.There were significant differences between different positions(P<0.05)under 5Hz.In the“V support ” the increase value of the surface electromyography muscle was the largest.Conclusions: Whole body vibration training become an effective intervention for chronic low back pain.It is suitable for patients who can not do strength training independently.Different vibration stimulation conditions has different effects on the lumbo-abdominal major muscles in patients with chronic lower back pain.The best frequency of lumbo-abdominal muscles is 15 Hz,The best exercise for multifidus muscles is plank.The best exercise for erector spinae muscles is plank.The best exercise for external obliqueis single bridge.the best exercise for rectus abdominison the vibration platform is“V” crunch.
