| This article consists of two parts:literature review and clinical research.Literature review:Including two parts, respectively discussing chronic obstructive pulmonary disease and acupoint sticking therapy. The first part of the review summarizes the research progress of Chronic Obstructive Pulmonary Disease from two aspects:Chinese medicine and Western medicine. The pathogenesis and treatment of chronic obstructive pulmonary disease are discussed in the Western medicine part.Chinese medicine elaborates the research progress of chronic obstructive pulmonary disease from the perspectives of etiology, pathogenesis, treatment of prescriptions of ancient and modern TCM doctors. The second part of the review discusses the research progress of acupoint patching therapy, from the historical evolution, theoretical basis, principle of action, acupoints selection and clinical application.Clinical research:Objective:To evaluate the clinical efficacy of combining acupoint patching therapy with conventional western medicine in the acute exacerbation of chronic obstructive pulmonary disease(external cold and fluid retention syndrome). And to explore the communication of lung and bladder theory, and provide its clinical verification. Methods:Refer to the 2017 GOLD guidelines for the diagnosis of acute exacerbations of chronic obstructive pulmonary disease and chronic obstructive pulmonary disease, and the diagnostic criteria of "external cold and fluid retention"syndrome in the 2011 Guidelines for TCM Diagnosis and Treatment of Chronic Obstructive Pulmonary Disease.Patients who met the inclusion criteria were randomly divided into the treatment group and the control group. The treatment group was treated with conventional Western medicine plus acupoint patching therapy. The control group was treated with conventional Western medical treatment. The course for both groups was 10 days. The symptom score form was recorded, regular blood tests and pulmonary function tests were taken before and after treatment.Observed indicators:1.Regular blood test(WBC,NE%,CRP).2.Pulmonary function test(FEV1,FVC,FEV1/FVC%).3.Symptom score(TCM syndrome score,CAT,mMRC,COPD clinical symptoms and signs score)4.COPD main symptoms and urinary conditions(cough, expectoration, dyspnea, urination and nocturia). Results:According to the inclusion criteria,62 acute exacerbation of chronic obstructive pulmonary disease patients were included in the study.2 patients were dropped, therefore a total of 60 patients, including 30 in the treatment group and 30 in the control group. There was no statistical difference between the two groups before treatment, and they were comparable. The total effective rate of clinical symptoms improved after treatment in the treatment group was 93.3%,and the total effective rate in the control group was 86.7%,P>0.05.There was no statistical difference in the result. The control group and the treatment group received regular blood tests, pulmonary function tests and TCM syndrome evaluation before and after treatment. The scores of clinical symptoms and signs,CAT,and the improvement of cough, expectoration, and dyspnea were different between both groups,P<0.05,indicating that the two groups were significantly improved after treatment.FEV1,FVC,TCM syndrome scores,CAT,COPD clinical signs and symptoms scores, cough, expectoration, urination were different between the two groups,P<0.05,indicating that the treatment group is superior than control group,to the improvement of COPD symptoms, pulmonary function and urination.Conclusion:(1)Conventional Western medicine treatment + acupoint patching therapy can improve the clinical symptoms of COPD patients compared with Western medicine treatment alone.(2)In the improvement of pulmonary function, conventional Western medicine treatment+acupoint patching therapy is better than Western medicine alone;(3)There may be a certain correlation between urinary condition and pulmonary symptoms. The theory of "lung and bladder communication" has its scientific and correctness and is of great significance for guiding clinical practice. |
