Objective:To explore the efficacy and safety of Yishen Cupai Decoction combined with acupuncture in the treatment of infertility caused by kidney deficiency and blood stasis type LUFS,and to evaluate it to guide clinical practice. Methods:Sixty patients with this disease were equally divided into two groups at random:Chinese medicine combined with acupuncture group(experimental group),Western medicine group(control group). The patients in the experimental group began to monitor ovulation on the 10th day of menstruation by Doppler ultrasound every other day. When the follicle grew to more than 15mm,patients began to take the Yishen Cupai Decoction(chuanniuxi, fupenzi, cubiejia, chaihu, liujinu, jixueteng, chishao, danggui, etc)each pack 200ML and twice one day,At the same time acupuncture treatment, every other day, alternating abdomen and back. When the ovulation grew to more than 18mm,monited every day until discharge, stop acupuncture and direct coitus, discontinue medication 5 days after ovulation. After 14 days, checking blood HCG or urine HCG determining whether or not pregnancy.Control patients from the 10th day menstruation began to monitor ovulation every other day. When the follicle grew up to more than 18mm,injecting the HCG 10000IU,and intercourse the next day,and after injecting HCG 48 hours confirmed whether follicles discharged by Doppler ultrasound. After 14 days, checking blood HCG or urine HCG determining whether or not pregnancy. If the follicle of 18mm in diameter or greater than three, cancelled this HCG injection cycle.avoided multiple births or ovarian hyperstimulation syndrome(OHSS).Efficacy indicators:ovulation rate and pregnancy rate after treatment, the changing of TCM syndrome points after treatment, and LUFS occurrence rate were observed in the two groups.Safety indicators:blood routine, urine routine, liver and kidney function, examination before and after the trial, occurrence of ovarian hyperstimulation syndrome(OHSS)during the treatment period. Results:At the end of the study, there were 30 cases in the experimental group with a total effective rate of 96.7%,and 28 cases in the control group with a total effective rate of 92.9%.The difference between the two groups was statistically significant(P=0.000<0.05).The ovulation rate of the experimental group was 89.77%,and that of the control group was 79.75%,the difference was not statistically significant(P=0.084>0.05),but the ovulation rate of the experimental group was higher than that of the control group.Pregnancy rate of the experimental group was 47%,and that of the control group was 14%.The difference between the two groups was statistically significant(P=0.015<0.05),and the pregnancy rate of the experimental group was significantly higher than that of the control group. The recurrence rate of LUFS in the experimental group was 9.09%,and that in the control group was 20.25%.The difference between the two groups was statistically significant(P=0.024<0.05),and the recurrence rate of LUFS in the experimental group was significantly lower than that in the control group. Conclusion:The ovulation rate of kidney-deficiency and blood-stasis type LUFS infertility treated with Yishen Cupai decoction combined with acupuncture showed no significant difference compared with that of HCG injection, but the pregnancy rate was significantly higher than that of the control group, which could improve the TCM syndromes of patients and significantly reduce the recurrence rate of LUFS.Combined with acupuncture,kidney-nourishing and blood-activating decoction had the fundamental treatment of tonifying kidney and promoting blood circulation, soothing liver and promoting qi,resolving phlegm clearing damp, making qi and blood harmonious, unobstructing meridians, and improving patients'symptoms and signs. The overall effect was better than the simple application of HCG.A history of endometriosis and pelvic infection had been shown to increase the incidence of LUFS.No adverse events occurred after treatment.Therefore,kidney-tonifying decoction combined with acupuncture also had a high clinical safety.
