Purpose:To investigate the effect of diosgenin from Dioscorea nipponica extract on the levels of some inflammatory factors in serum of rats with painful diabetic peripheral neuropathy and its mechanism.It provides a new method and idea for the prevention and treatment of painful diabetic peripheral neuropathy with Chinese herbal medicine,and provides a deep theoretical basis for the optimal choice of clinical diagnosis and treatment drugs.Material and method:Select 136 SPF male Wistar rats,weight 180-200 g,adaptability to feed a week later,randomly selected from 24,it is divided into three groups,each group of 8,respectively as the second week,the fourth week,eighth week normal control group.The remaining 112 rats were injected with 2% Streptozotocin(STZ)53 mg/kg(0.1 mol/l citric acid buffer of PH4.2)in a single intraperitoneal injection.Rats with blood sugar greater than 16.7 mmol/L were selected and randomly divided into model control group,high-dose diosgenin group,low-dose diosgenin group and prednisone group.The mental state and daily activities of rats at 2,4 and 8 weeks after administration were observed.The effects of diosgenin on the expression of IL-6,VCAM-1,VEGF and adiponectin in serum of rats with PDPN were detected by ELISA.Results: 1.The mental state of model group rats,appear more drinks,food,urine with apparent angular,the colour faded lacklustre,nuzzles back arched,slow to wait for a symptom,the treatment group rats are appear afore-mentioned symptoms,but the degree of light in the model group,the yam saponins in high dose group and prednisone group rats symptoms improved relatively obvious.2.The blood glucose level of the model group was significantly higher than that of normal rats(P < 0.01).There was no statistical difference between the blood glucose level and the model group in the treatment group(P > 0.05).3.The motic nerve conduction velocity(MNCV)in the model group was significantly lower than that in the normal group(P < 0.01).The motic nerve conduction velocity(MNCV)was significantly increased in the treatment group compared with the model group(P < 0.01).4.The pain threshold of the model group was significantly higher than that of normal group rats(P < 0.01).In 2 weeks,the pain threshold of diosgenin in high dose group,prednisone group and model group was lower than that in rats(P < 0.05).At 4 weeks,8 weeks,the treatment group compared with model group significantly reduced the pain threshold of rats(P < 0.01),Chinese yam saponins in low dose group and high dose group compared with the pain threshold of rats increased significantly(P < 0.01),prednisone and saponin content of high dose group compared to raise the pain threshold of rats(P < 0.05),prednisone and saponin content low dose group compared with the pain threshold of rats reduced(P < 0.05).5.Compared with the normal group,the expression levels of IL-6 and VCAM-1 in serum of rats in the model group increased significantly(P<0.01),and the expression levels of vascular endothelial growth factor and adiponectin decreased significantly(P<0.01);compared with the model group,the expression levels of IL-6 and VCAM-1 in serum of rats in the treatment group decreased significantly(P<0.01),and the expression levels of vascular endothelial growth factor and adiponectin increased significantly(P<0.01).At 2 weeks,the levels of IL-6 and VCAM-1 in serum of rats in low-dose group,prednisone group and high-dose group were significantly higher than those in high-dose group(P < 0.01),while the levels of expression of vascular endothelial growth factor and adiponectin were significantly lower(P < 0.01).At 4 weeks,the levels of serum vascular endothelial growth factor(VEGF)and adiponectin in low-dose group,prednisone group and high-dose group were significantly lower than those in high-dose group(P < 0.01),and the levels of IL-6 and VCAM-1 were significantly higher than those in prednisone group(P < 0.01),and the levels of IL-6 and VCAM-1 in low-dose group were significantly higher than those in prednisone group(P < 0.01),while the levels of VCAM-1 were significantly lower(P < 0.01).At 8 weeks,the levels of serum vascular endothelial growth factor and adiponectin in the low-dose group,prednisone group and high-dose group were significantly lower than those in the high-dose group(P < 0.01).The levels of IL-6 and VCAM-1 in the high-dose group and prednisone group were significantly higher than those in the low-dose group(P < 0.01).Conclusion: Diosgenin from Dioscorea nipponica extract can inhibit the development of inflammatory reaction in serum of diabetic rats,alleviate pain symptoms of painful diabetic peripheral neuropathy rats,and improve nerve damage of painful diabetic peripheral neuropathy rats.
