Observation On The Curative Effect Of Shenlingbaizhusan In Treating Femoral Neck Fracture With Spleen Deficiency And Dampness Syndrome After Total Hip Arthroplasty

Objective:to observe the clinical effect of Shenlingbaizhu powder in the treatment of Wet Syndrome due to spleen deficiency after total hip replacement(THR)for femoral neck fracture,and to explore the mechanism,so as to provide a clinical treatment plan for rapid rehabilitation after THR.Methods:From November 2018 to October 2019,50 elderly patients with spleen deficiency and Dampness Syndrome underwent unilateral total hip arthroplasty in six departments of bone injury of Changsha Hospital of traditional Chinese Medicine(South Hospital)due to femoral neck fracture,the patients were divided into treatment group(25 cases)andControl Group(25 cases).Treatment Group:The first day after the operation to Shenlingbaizhu decoction oral,14 days course of treatment.Control Group:Basic Treatment after total hip replacement.The course of treatment is 14 days.The symptom scores,CD3+,CD4+,CD8+,CD4+/CD8+,HB and Harris hip scores,Therapeutic effect score of TCM Syndrome were observed and compared between the two groups.Results:There was no significant difference in preoperative data between the two groups(p>0.05).On the first day after operation,the TCM symptom scores of the treatment group and the Control Group were statistically significant(p<0.05).The TCM symptom scores of the treatment group and the Control Group on the 3rd,7th and 14th day after operation were statistically significant(p<0.05).On the 7th and 14th day after operation,the CD3+of the treatment group and the control group were significant(p<0.05).On the 14th day after operation,CD4 + in the treatment group was significantly higher than that in the control group(p<0.05).There was no significant difference in CD8+ between two groups(p>0.05)before operation,1 day after operation,7 days after operation and 14 days after operation.The CD4+/CD8+ ratio of the two groups on the 14th day after operation was significant(p<0.05).On the 3rd,7th and 14th day after operation,there was no significant difference in HB between the Treatment Group and the control group(p>0.05).The Harris score of 14 days after operation and 1 month after operation was significantly improved(p<0.05).The curative effect of Tcm Syndrome in the treatment group and the Control Group 14 days after operation was statistically significant(p<0.05).There were 3 DVT cases in the treatment group and 6 DVT cases in the control group.Conclusion:1.Shenling Baizhu powder can effectively improve the immune function of elderly patients,and its mechanism is realized by improving CD3+,CD4+and CD4+/CD8+.2.Shenlingbaizhu powder has no clinical significance in treating anemia caused by recessive blood loss after total hip replacement3.Harris hip score of 14 days and 1 month was better than that of the Control Group,which suggested that Shenlingbaizhu powder could improve the function of hip joint.
