[Purpose] The family is the basic place for residents to participate in social activities and the basic unit for residents to make collective decisions.The purpose of this article is to analyze the health service needs of empty-nest families in rural areas at the family level,use the framework of field theory to explore theoretical models,and use family collection models for empirical analysis.[Methods] In this study,a multi-stage random sampling was conducted through the field survey method,2 counties were selected in 2 provinces,5 towns were selected in each county,40 households in 6 villages were selected in each town,and2715 households with 7293 residents were finally selected.803 empty nest families and 1,606 residents conducted health service surveys.Through a questionnaire survey,residents were asked about the demand for medical services,public health and other services.Using field theory to construct a theoretical model of health service demand based on family units,and according to the needs of family members to answer the agreement,calculate the consistency rate of the health service demand of empty nest families,and discuss the feasibility of health service demand based on family units.Finally,the empirical test of the family collection model is used to obtain the health service needs of the empty nest family residents at the family level.[Results](1)Theoretical model results: According to field theory,family field capital mainly includes economic capital,cultural capital and symbolic capital.Due to different family-led capitals,different capital-led demand models are constructed separately.Economic capital in the family field mainly involves the economic level of the family.Cultural capital is mainly reflected in the internal culture of the family,community culture and regional culture.Symbolic capital is mainly the role of family members in the family.Different capitals have different degrees of health needs for members Impact.(2)Demand consistency: Residents of empty nest families have consistent health service demands,which provide good support for family health service demands.Except for the diagnosis and treatment services,family doctors and other service demands are greater than 50%,of which the Internet medical demand consistency is96.0%,the incidence of hypertension is 89.1%,and the proportion of mental health services is 72.5%.According to the results of the chi-square test,hypertension in empty-nest families with low education level gradually leads to a degree of demand consistency(χ~2 = 3.465,P <0.1).The family doctors of empty nest families with good health level have a higher consistency of mental health service demand(χ~2 = 4.542,P<0.1,χ~2 = 7.803,P <0.05).4.461,P <0.05).The male-headed households have a higher consistency of mental health service demands(χ~2 = 5.982,P <0.05).(3)Empirical model results: According to the family set theory,under the comprehensive role of family field theory,the results show that: 1)The ratio of the common demands of members to the health demands of high-level households is93.4%,and the main demands are medical services(43.3 %),Family physician services(39.0%)and health education services(24.9%).Among them,94.0% of the households are male-headed households,69.3% of the households have at least one member sick within two weeks,and 82.4% of the household members have chronic diseases.The results show that 2)The proportion of the demands of one of the members as family-level demands is 5.7%,the content of the demand is medical services(56.5%),medical examination services(65.2%)),family doctor services(58.7%),Health education services(63.0%)and demand for Chinese medicine services(45.7%).If one of a member’s demands are the collective demands of the family,the member has a disease prevalence(100.0%)within two weeks,or suffer from chronic diseases(82.6%).[Conclusions] There are obvious family characteristics in the health service demands of empty-nest families in rural areas of China.Under different family fieldcapitals,the demands of empty nest family health services vary,but on the whole,the main demands include diagnosis and treatment services,physical examination services,family doctor services,health education services and traditional Chinese medicine services.Different from the individual-based health service requirements,there is a certain degree of consistency in the health service demands of the empty nest family members and the demands of the members affect each other.The content of the demands is mainly public health services.In addition,since empty nest families lack sufficient capital and can only meet the basic health demands of families,they should pay attention to the individual demands of members on the basis of meeting their common demands,and at the same time pay attention to empty nest families "poverty due to illness".The phenomenon of "returning to poverty due to illness".In general,research on family-based health demands provides new ideas for integrated services,provides evidence-based evidence for the establishment and improvement of the family-based health service supply model,and has an important reference for improving the efficiency of rural primary health service supply systems.Significance,providing a scientific basis for family doctors and other family-based service delivery models.[Innovation and Limitation] First,unlike the previous research on the demands of personal health services,this research innovatively proposes a model of health service demand based on households;second,there are few previous studies on family-based units,and Research on the demand for family health services is even more scarce.Thirdly,this paper innovatively uses the field theory to construct a family health service demand theoretical model,and conducts an empirical test with the family collection model.However,there are still deficiencies in this study.Only the narrow-nest empty-family family of two-person families is taken as the research object,and other types of families such as single-person families and intergenerational families are not considered.
