| OBJECTIVE:In view of the fact that the incidence and prevalence of type 2 diabetes continue to rise around the world,early targeted screening is particularly important.CD36 has been proved to be involved in a variety of metabolic responses.As a form of CD36acellular binding cycle,s CD36 can reflect the expression of CD36 in tissue.In order to detect,prevent and reduce diabetes-related complications,we explore the relationship between s CD36 and glucose and lipid metabolism in patients with type 2diabetes with normal renal function,and summarize its potential role in predicting diabetes and its complications.METHODS:A cross-sectional study was conducted on 295 T2DM patients with normal renalfunction in the Department of Diabetic Nephrology,Tianjin Medical University Chu Hsien-I Memorial Hospital from 2018.04 to 2019.12.According to the level of Hb A1c,they were divided into three groups:group A(95 cases,male 51,female 44):Hb A1c≤7.0%;group B(130 cases,male 70,female 60):7.0%≤Hb A1c≤9.0%;group C(70,male 44,female 26):Hb A1c>9.0%.RESULTS:1.Comparison of clinical data of patients with different groups of Hb A1cThere were no significant differences in gender composition,age,disease course,body mass index,systolic blood pressure,diastolic blood pressure,serum albumin,alanine aminotransferase,glutamate aminotransferase,glomerular filtration rate,serum uric acid and HDL cholesterol between the three groups.Compared with the group A,triglycerides increased in group B and group C(p<0.05).Compared with group B and group C,the concentrations of total blood cholesterol and LDL cholesterol in group C were increased(p<0.05).After adjusting for the effect of differences in blood total cholesterol and LDL cholesterol concentrations,it was found that serum s CD36level was increased in group C(p<0.05),compared with group A and group B(p<0.05).2.Comparison of insulin resistance and insulin islet secretion in patients with different Hb A1c levelsCompared with group A,HOMA-IR was increased in group B for evaluating insulin resistance and ISI for evaluating insulin sensitivity,with statistically significant differences(p<0.05).HOMA-βof group B and difference group C who evaluated basic insulin secretion function decreased,and the difference was statistically significant(p<0.05).HOMA-βalso had statistical significance between the two groups(p<0.05).Compared with group A and group B,the level of FINS secretion in group C was reduced,and the difference was statistically significant(p<0.05).3.Correlation and regression analysis between serum soluble CD36 and related indexess CD36 was negatively correlated with FINS,HOMA-IR and HOMA-β,and positively correlated with the concentration of TC,LDL-C,TG and Hb A1c.Taking the above related indicators as independent variables and s CD36 as dependent variables,and takingα<0.05 as criteria for stepwise regression screening variables,multiple linear regression analysis was adopted and stepwise method was selected.Multiple stepwise regression analysis showed that FINS and LDL-C,were negatively correlated with FINS and positively correlated with LDL-C.The results showed that FINS and LDL-C,had negative correlation with FINS and positive correlation with LDL-C.4.Correlation and regression Analysis between Serum s CD36 concentration and related Indexes in patients with different Hb A1cIn group A,s CD36 was negatively correlated with FINS,HOMA-IR and HOMA-β,and positively correlated with TC,HDL-C,LDL-C and ISI.Multiple stepwise regression analysis with the above-mentioned related indexes as independent variables and s CD36 as dependent variables showed that HOMA-IR,was positively correlated with TC and ISI,while in group B,s CD36 was negatively correlated with FPG,FINS,HOMA-IR,HOMA-βand positively correlated with ISI.Multiple stepwise regression analysis with the above-mentioned related indexes as independent variables and s CD36 as dependent variables showed that the main variables affecting s CD36 were negative correlation with HOMA-IR,while in group C,s CD36 was negatively correlated with BMI,UA,FINS and HOMA-β,and positively correlated with HDL,Hb A1c and FPG.Multiple stepwise regression analysis with the above-mentioned related indexes as independent variables and s CD36 as dependent variables showed that s CD36 was negatively correlated with HOMA-βand positively correlated with Hb A1c.CONCLUTION:1.The level of s CD36 is affected by glucose and lipid metabolism.2.The level of s CD36 was correlated with the control degree of glucose metabolism.When the blood glucose was poorly controlled(Hb A1c>9.0%),the level of s CD36was increased.3.Fasting insulin level is related to a variety of factors.Under the influence of glucose toxicity,the higher the blood glucose is,the worse the insulin secretion function is.4.When blood glucose was well controlled(Hb A1c≤7.0%),the level of s CD36 was mainly affected by lipid metabolism.When blood glucose control was poor(Hb A1c>9.0%),glucose metabolism had a significant influence on s CD36 level. |
