| Objective:The effects of different doses of remazolam anesthesia on sedation depth and hemodynamics in elderly patients provided a theoretical basis for the better application of remazolam in general anesthesia induction in elderly patients.Methods:100 elderly patients with ASA grade I or II elective surgery requiring tracheal intubation were selected,and the patients were divided into 4 groups using a random number table method.Remazolam group(experimental group): groups A,B,and C were induced with different doses of remazolam,the doses were 0.25 mg/kg,0.3mg/kg,and 0.35 mg/kg;D group(control group)was induced by propofol and etomidate,After BIS was less than 60,rocuronium of 0.6 mg/kg and sufentanil of 0.4ug/kg were injected intravenously.After muscle relaxation was completed,the intubation was completed and mechanical ventilation was performed.Record the time from induction to BIS ≤ 60,the BIS value during intubation and the hemodynamic indexes such as heart rate(HR),invasive arterial blood pressure(SBP、DBP)and serum norepinephrine(NE)values were recorded at 3 time points before induction(T0),the start of induction to BIS ≤ 60(T1),and 4 min(T2)after tracheal intubation.At the same time,record injection pain,bradycardia,tachycardia,hypertension and low blood pressure and other adverse events.Results:1)Comparison between remazolam group(A,B,C)and group D: the time from induction to BIS ≤ 60,group D>group A>group B>group C,the difference is statistically significant(p<0.05),group D was the slowest,group C was the fastest;Hemodynamics indexes and norepinephrine of remazolam group were higher than those of group D at T1 time point,the difference was statistically significant(p<0.05).The incidence of injection pain,hypotension,bradycardia and other drugs in group D was higher than that of remazolam group,and the difference was statistically significant(p<0.05).2)Comparison between remazolam groups(A,B,C): The A group had 3 cases of sedative drug remediation(p<0.05),and the C group had 2 cases of hypotension(p<0.05).3)The heart rate of the remazolam group at T1 and T2 was higher than T0,the difference was statistically significant(p<0.05);the average arterial pressure and norepinephrine at T0 and T2 were higher than T1,the difference was statistically significant(p<0.05).4)The T0 and T2 hemodynamics and norepinephrine concentration indexes of group D were higher than those of T1,and the difference was statistically significant(p<0.05).Conclusion:Remazolam of 0.3 mg/kg is used for general anesthesia induction in elderly patients.The induction is relatively fast,the hemodynamics are stable,the adverse reactions are few and the safety is high,and it is more suitable for elderly patients. |
