| Objective:1.To translate the Kihon Checklist(KCl)into Chinese and evaluate the reliabili ty and validity.2.Apply the Chinese version of Kihon Checklist(KCl)to the elderly in the co mmunity,and get the critical score for the elderly in the community.Methods:The study consists of two parts: the first part is to translate the Kihon Checklist(KCl)into Chinese and test reliability and validity.The second part is to use the Chinese version of Kihon Checklist(KCl)to screen the critical score of the elderly in the community.The first part is to translate the Kihon Checklist(KCl)into Chinese.The scale is published on the website of the health and labor of Japan,and can be downloaded from the website for free,and does not involve intellectual property disputes.This study will strictly follow the Brislin translation model and translate it into Chinese by using the following methods: sequential translation,back translation,cultural adaptation and pre experiment.A total of 458 elderly people over 60 years old of 6 communities in Taiyuan city were selected to conduct a formal survey.Reliability was evaluated by Cronbach’s α coefficient and retest reliability;Validity use content validity and construct validity evaluation;factor analysis was used to test the structural validity of the scale.The second part is a cross-sectional study.308 elderly people over 60 years old were selected from six communities in Taiyuan City,Shanxi Province.General information questionnaire,Fried phenotype and Chinese version of KCl scale were used for screening.With Fried phenotype as reference,sensitivity,specificity and ROC curve were used to evaluate the Chinese version of KCl scale,and the appropriate critical value of frailty was obtained.Spss24.0 was used to analyze the data.The general situation of the research object is represented by the average value,constituent ratio,frequency,standard deviation.Sensitivity,specificity,Youden index and ROC curve were used to describe the accuracy of the tool.Results:Part of the first study has formed a screening scale which is in line with the cultural background of our country.The Chinese version of KCl scale includes seven dimensions:functional activities of daily living(6 items),physical activity status(5 items),nutritional status(2 items),oral function(3 items),social status(2 items),cognitive ability(3 items)and emotional status(3 items).The total Cronbach’s α coefficient of the scale was 0.882,and the Cronbach’s α coefficient of each dimension was 0.733-0.856.The Cronbach’sαcoefficient of the total test-retest reliability was 0.944,the Cronbach’s α coefficient of each dimension was 0.902-0.979,the I-CVI of each item was 0.86-1,and the S-CVI/Ave of the total content validity was 0.93.Exploratory factor analysis showed that the load value of each item was > 0.4 in its dimension.According to the structure of the original scale,the confirmatory factor analysis showed that the factor model fit well,the factor load values were > 0.4,the fitting index x2/df = 2.388,RMSEA = 0.067,CFI = 0.905,TLI = 0.884,IFI = 0.906,NFI = 0.848.The second part of the results showed that When the cut-off value of Chinese version of KCl was 7,the maximum area under the ROC curve(AUC)was 0.929,the sensitivity was 0.908,the specificity was 0.782,and the Youden index was 0.690,which was consistent with the Fried phenotype,and the kappa value was 0.545,indicating that the scale has good application prospects and screening value.Conclusions:1.The Chinese version of Kihon frailty screening scale has good reliability and validity,and can be used to assess frailty of the elderly in the community.2.Using the Chinese version of the kihon frailty screening scale to assess the frailty of the elderly in the community,we can get the cut-off point of the Kihon elderly frailty screening scale suitable for the elderly in the community,which enriches the frailty screening tools for the elderly in the community,and also provides some basis for targeted intervention. |
