In the age of population aging and health inequality,studying the health status of different groups participating in basic medical insurance for middle-aged and elderly residents and the causes of inequality are helpful to analyze the causes of health inequality,In order to narrow the health gap between the research on the subject,change the mismatch of the health sector among urban and rural study groups evaluate the utility of residents’ basic medical insurance and perfect the urban and rural medical insurance pooling mechanism to provide an objective basis.First of all,the concept and measurement of health-related quality of life(QOL)were defined by referring to domestic and foreign literature and accumulating relevant theoretical basis,and the health concentration index was used as an indicator to measure health equity.On the one hand,a questionnaire was designed to measure the health-related quality of life of middle-aged and elderly residents in China based on the survey concept of the European Five-dimensional Health Scale,and the contents of the questionnaire were filled with data from the 2018 CHARLS health survey for middle-aged and elderly residents to prepare for the follow-up empirical research.On the other hand,through the basic characteristics of demography,economic level,individual social factors and other variables of statistical description to analyze the urban and rural middle-aged and elderly residents health differences.The results show that the average health level of middle-aged and elderly population in rural areas is obviously worse than that of urban residents.Middle-aged and elderly people with high economic level enjoy higher health advantages;Higher education gives middle-aged and elderly people more opportunities to enjoy and maintain health advantages;Middle-aged and elderly people with more children performed poorly in health-related quality of life.On the basis of clarifying the general direction of the health gap between the middle-aged and the elderly in urban and rural areas,the CEM matching method was used to screen the samples twice,and the middle-aged and the elderly in the samples were classified based on the basic medical insurance of insured residents.Secondly,based on the above data and empirical studies,an empirical study on the health level and equity of middle-aged and elderly people under residents’ basic medical insurance is prepared.The first step is to clarify the health effects of the three types of basic medical insurance on middle-aged and elderly residents by eliminating the influence of confounding factors after matching.In the second step,the health effects of the control variables and the explanatory variables on the classified samples were obtained based on the analysis framework of the health linear regression equation and the reasons were analyzed.The third step is to measure the degree of health inequality among different groups by insurance types and decompose the obtained results,so as to clarify the contribution of different factors to the current health inequality among middle-aged and elderly groups.The estimated results show that the overall basic medical insurance for urban and rural residents has no obvious effect on the health improvement of the middle-aged and elderly population,the basic medical insurance for urban residents has significantly improved the health level of the insured group,and the new rural integration has not improved the health level of the middle-aged and elderly population.The three types of basic medical insurance all improve the health equity among the insured subjects;The economic level gap is the leading factor that causes the health disparity and the health inequality;Influenced by education level,number of children and working status,the performance of health level and inter-group health equity of different insured samples in urban and rural areas may be different.Finally,based on the qualitative and quantitative analysis above,this paper puts forward targeted suggestions for improving the health level and health equity of middle-aged and elderly groups,so as to provide suggestions for the reform of the national medical and health system.Finally,based on the above qualitative and quantitative analysis,this article puts forward specific suggestions for improving the health level and health equity of the middle-aged and elderly groups,so as to provide suggestions for the reform of the national medical and health system.
