| Due to the impact of China’s urban-rural dual economic structure,urban and rural medical insurance has been operated separately,which has led to many problems,such as repeated participation in the insurance,lack of fairness,etc.,making medical insurance reform inevitable.In 2016,the State Council issued the guidance document "Opinions on the Integration of the Basic Medical Insurance System for Urban and Rural Residents" on the integration of the new rural cooperative medical care system and urban residential insurance,gradually changing the situation of the basic medical insurance system divided between urban and rural areas.Shangqiu in the related policy under the guidance of the work of system integration,to start on January 1,2017 the implementation of urban and rural residents basic medical insurance,the implementation of the integration of urban and rural residents basic medical insurance,broke the urban and rural medical insurance binary boundaries,making health care coverage is more extensive,urban and rural residents to enjoy the same medical treatment.This paper takes the integration of basic medical insurance for urban and rural residents in Shangqiu as the research object,and takes welfare economics,dual economic structure,customer satisfaction and other theories as the support.Under the conditions of sorting out literature,collecting relevant data and field investigation and visit,The problems existing in the integration of basic medical insurance for urban and rural residents in Shangqiu were analyzed from the perspectives of urban and rural residents,medical and health institutions and the government by case analysis and statistical analysis.Secondly,it analyzes the differences between urban residents and rural residents in the evaluation of government subsidy amount,reimbursement threshold,reimbursement process and so on.Finally,the multivariate ordered Logistic model was used to analyze the factors affecting urban and rural residents’ satisfaction with the basic medical insurance from seven aspects,such as the degree of cognition of the insured residents on the basic medical insurance for urban and rural residents.The results show that: First,the cultural and income gap between urban and rural residents in Shangqiu is large,and there is a lack of understanding of medical insurance.There is a large gap between urban and rural medical resources,high pressure on upper medical institutions,drug price is on the high side,and the existence of induced consumption.Lack of professionals,lack of medical insurance services,medical insurance fund raising and payment pressure,the level of medical security needs to be improved.Second,in terms of the amount of government subsidies,urban residents have a higher evaluation than rural residents,while in terms of whether to reduce the medical burden,rural residents have a higher evaluation.Third,the choice of medical treatment after illness,whether the designated medical institutions have induced consumption,the amount of subsidies,and whether the medical burden is reduced are the factors that affect the satisfaction of urban and rural residents with basic medical insurance.In view of the above mentioned problems and analysis,this paper puts forward suggestions from three aspects: improving the development environment of basic medical insurance for urban and rural residents,giving full play to government functions and perfecting the medical security system,in order to provide reference for the integrated implementation of urban and rural medical insurance in Shangqiu. |
