| Objective: The purpose of this study was to evaluate the concentration le vel of ICAM-1 and progranulin in BLAF with interstitial lung disease.Explor e the relationship between factors and ILD patients to provide references for cl inical application.Methods:Clinical data of 93 confirmed ILD patients admitted in the Second Hospit al of Hebei Medical University between September 2018 and September 2020,were retrospectively analyzed.All patients were divided into three groups:1)I IP group,Including IPF(n=21),IPAF(n=21)and other conditions(n=5);2)CT D-ILD group,Including PM/DM-ILD(n=14),RA-ILD(n=12),SS-ILD(n=8)a nd other conditions(n=8);3)Other types of ILD group.Collect the arterial gas analysis,pulmonary function indexes,high-resolution tomography of chest,6-minute walking test distance.All patients were examed with bronchoscopy.T he level of ICAM-1 and progranulin in broncho alveolar lavage fluid was mea sured by ELISA.Results:1.Expression levels of ICAM-1 and progranulin:The ICAM-1 levels of IIP group and CTD-ILD group were 7.55±5.70ng/ml and 8.59±5.84 ng/ml(P>0.05);The progranulin levels of IIP group and CT D-ILD group were 78.67±153.31ng/ml and 159.91±202.31ng/ml,respectively(P=0.034).2.Correlation analysis of ICAM-1 expression levels in BALF with indexe s:2.1 The concentration of ICAM-1 in BALF of IPF patients was negativel y correlated with distance of 6MWT,OI,FVC% predicted,DLCO% predicted (r=-0.469,P=0.043;r=-0.732,P=0.000;r=-0.662,P=0.002;r=-0.572,P=0.010).Meanwhile,it was positively correlated with HRCT point(r=0.712,P=0.000).2.2 The concentration of ICAM-1 in BALF of IPAF patients was negativ ely correlated with distance of 6MWT,OI,FVC% predicted,DLCO% predict ed(r=-0.538,P=0.021;r=-0.733,P=0.000;r=-0.798,P=0.000;r=-0.761,P=0.000).Meanwhile,it was positively correlated with HRCT point(r=0.655,P=0.001).2.3 The concentration of ICAM-1 in BALF of CTD-ILD patients was neg atively correlated with OI,FVC% predicted,DLCO% predicted(r=-0.640,P=0.000;r=-0.458,P=0.007;r=-0.549,P=0.001).Meanwhile,it was negatively c orrelated with FVC% predicted(r=-0.458,P=0.007).3.Correlation analysis of progranulin expression levels in BALF with ind exes:3.1 The concentration of PGRN in BALF of IPF patients was negatively correlated with OI,FVC% predicted,DLCO% predicted(r=-0.549,P=0.010;r=-0.569,P=0.011;r=-0.659,P=0.002).Meanwhile,it was positively correlate d with HRCT point(r=0.514,P=0.017).3.2 The concentration of PGRN in BALF of IPAF patients was negativel y correlated with OI,FVC% predicted,DLCO% predicted(r=-0.646,P=0.002;r=-0.585,P=0.011;r=-0558,P=0.016).Meanwhile,it was positively correlate d with HRCT point(r=0.439,P=0.023).3.3 The concentration of PGRN in BALF of CTD-ILD patients was negat ively correlated with OI,FVC% predicted,DLCO% predicted(r=-0.552,P=0.000;r=-0.449,P=0.009;r=-0.503,P=0.001).Meanwhile,it was positively cor related with HRCT point(r=0.579,P=0.000).Conclusions:1.The expression levels of ICAM-1 and PGRN were different in different types of interstitial lung disease,that can be used in the diagnosis and differen tial diagnosis of ILD.2.The expression levels of ICAM-1 and PGRN provided some indication of the disease severity. |
