| Objective: To explore the effectiveness of locking plate internal fixation(PHILOS)combined with rotator cuff repair and anterior acromioplasty through interspace approach between pectoralis major and deltoid in elderly patients with proximal humeral fracture complicated with rotator cuff injury and anterior acromion hyperplasia.Methods: Retrospective analysis of 80 elderly patients with proximal humerus fracture who underwent PHILOS combined with rotator cuff repair in Nanjing Qixia District hospital from January 2014 to December 2018.According to the operative method,they were divided into two groups:24 patients in the study group and 56 patients in the control group.The study group was treated with PHILOS,rotator cuff repair and anterior acromioplasty through interspace approach between pectoralis major and deltoid,while the control group was treated with PHILOS combined with rotator cuff repair.The excellent and good rate of shoulder joint function after treatment,pain score before operation,1 week,1 month and 3 months after operation were compared between the two groups.Results: After treatment,the excellent and good rate of the study group87.5% was higher than that of the control group66.0%(P < 0.05);the pain level of the study group in 1 week,1 month and 3months after operation(1.63±0.56、1.21±0.50、0.92±0.57)was significantly lower than that of the control group(2.71±0.65、2.20±0.55、1.88±0.54)(P <0.05).CONCLUSION: This study mainly explores the effectiveness of locking plate internal fixation(PHILOS)combined with rotator cuff repair and anterior acromioplasty through interspace approach between pectoralis major and deltoid in elderly patients with proximal humeral fracture complicated with rotator cuff injury and anterior acromion hyperplasia.The clinical therapeutic effect is very good.It is worthy of clinical application and further study. |
