China has entered an aging society since the beginning of the 20 th century.According to the latest census data,by 2020,China has 264 million elderly people aged 60 and over,accounting for 18.7% of the total population.On the one hand,in the context of population aging,the prevalence of the elderly is high,the number of disabled elderly is increasing,and the medical care needs of the elderly are expanding;On the other hand,with the increase of medical expenses,the "medical care instead of support" nursing model for the disabled elderly has brought a serious burden to the family and social medical security expenditure.In order to ease the financial burden of the medical insurance fund and the family of the elderly,Qingdao issued the Opinions on The Establishment of Long-term Medical Care Insurance System in 2012,which was the first to carry out the pilot work of long-term care insurance and embarked on the mode of combining medical care with nursing care.Then,in 2016,the human resources and Social Security Bureau issued the guidance on the pilot of long-term care insurance system,and 15 cities such as Shanghai and Chengdu were gradually included in the pilot scope of long-term care insurance.With the increasing number of pilot cities,the long-term insurance system has achieved preliminary results.Long term care insurance can affect people’s medical consumption through the compensation of nursing expenses.But theoretically,the direction of this influence is uncertain.Therefore,this paper analyzes whether long-term care insurance can reduce the pressure of medical consumption of elderly groups,and comprehensively discusses the impact of long-term care insurance on medical consumption of elderly groups.Firstly,the article defines the relevant concepts and analyzes the relevant factors affecting medical consumption,so as to provide a theoretical basis for the study of the impact of long-term care insurance on medical consumption.Secondly,through the analysis of the current situation,this paper briefly introduces the development of long-term care insurance pilot in China,and compares and analyzes the differences of long-term care insurance policies in various cities in terms of coverage groups,financing methods,payment treatment and so on.This paper analyzes the current situation of medical expenditure and utilization of medical and health services for the elderly.Finally,in the empirical study,the article selects the survey data of Charls from 2011 to 2018,and selects five pilot cities of long-term care insurance:Qingdao,Shanghai,Jingmen,Suzhou and Shangrao.It uses the double difference method to construct a regression model to study the impact of long-term care insurance on the medical consumption of the elderly in China.The empirical results show that long-term care insurance can effectively reduce the medical consumption of the elderly,especially in the cost of hospitalization.Moreover,the impact of long-term care insurance on the medical consumption of elderly groups is different in different cities.Specifically,it has a significant negative impact on the medical consumption of elderly groups in Qingdao,Shanghai and Jingmen,which can effectively reduce the number of visits and hospitalization and medical expenses of local elderly groups;In Suzhou and Shangrao,the impact effect is not significant,and even positive.The article then explores the causes of regional differences in results,and makes a robustness test.Finally,based on the current situation and empirical analysis conclusions,this paper puts forward corresponding countermeasures and suggestions from the aspects of expanding the pilot of long-term care insurance,improving the long-term care insurance system and promoting the construction of long-term care service system,so as to provide support for the further development of long-term care insurance in China.
