Objective: To explore the clinical effect and the action mechanism of core training combined with bloodletting therapy in weizhong acupoint on the treatment of lumbar disc herniation of qi stagnation and blood stasis type.Methods: 120 patients who met the inclusion criteria were randomly divided into four groups:control group,bloodletting therapy group,core training group and combined treatment group.There were 30 patients in each group,and each group was treated for 4 weeks.In the first and second weeks,the control group was treated with usual therapy,sham bloodletting therapy in weizhong acupoint and functional exercise;the bloodletting therapy group was treated with usual therapy,bloodletting therapy in weizhong acupoint and functional exercise;the core training group was treated with usual therapy,sham bloodletting therapy in weizhong acupoint and core training;the combined treatment group was treated with usual therapy,bloodletting therapy in weizhong acupoint and core training.In the third and forth weeks,the the control group and bloodletting therapy group continued with functional exercise;the core training group and the combined treatment group continued with core training.Visual analogue score(VAS score),Japanese Orthopaedic Association Score(JOA score),lumbar flexion and extension activity were collected from each group before treatment,two weeks and three months after treatment.Then,through statistical analysis,the short-term and long-term effects of the four treatment methods were compared.Results:(1)The VAS score,JOA score,lumbar flexion and extension activity of four groups after two weeks and three months of treatment were significantly better than those before treatment(p < 0.05).(2)After two weeks of treatment,the therapeutic effects of the bloodletting therapy group were significantly better than those of the control group(p < 0.05);the JOA score and the lumbar flexion activity of the bloodletting therapy group were significantly better than those of the combined treatment group(p < 0.05);the therapeutic effects of the combined treatment group were significantly better than those of the core training group(p < 0.05);the JOA score and the lumbar flexion activity of the core training group were significantly better than those of the control group(p < 0.05).(3)After three months of treatment,the therapeutic effects of the combined treatment group were significantly better than those of the bloodletting therapy group(p < 0.05);the VAS score of the combined treatment group was significantly better than that of the core training group(p <0.05);the therapeutic effects of the core training group were significantly better than those of the the control group(p < 0.05).(4)After two weeks of treatment,the total efficacy on the control group,the bloodletting therapy group,the core training group and the combined treatment group was 80.00%,96.67%,86.67%,and 90.00%,respectively.After three months of treatment,the total efficacy on the control group,the bloodletting therapy group,the core training group and the combined treatment group was 76.67%,83.33%,93.33% and 96.67%,respectively.Conclusion:(1)The effect of bloodletting therapy in weizhong acupoint on lumbar disc herniation of qi stagnation and blood stasis type on early stage was remarkable.(2)Core training was effective in the treatment of lumbar disc herniation of qi stagnation and blood stasis type on middle-late stage and the recurrence rate was low.(3)The effect of core training combined with bloodletting therapy in weizhong acupoint on lumbar disc herniation of qi stagnation and blood stasis type on early and middle-late stage were prominent,with low rate of recurrence. |
