Background Malignant pleural effusion and tuberculous pleural effusi on are common pleural effusions.The incid ence rate of these two types of pleural effusion is high er.As a common complication of malignant tumor,malignant pleural effusion has high degree of malignancy,difficult clinical treatment and poor prognosis.Th erefore,early identification and diagnosi s of the nature of pleural effusion is an important condition to improve the prognosis of patients with malignant pleural effusion.Limited to the current situation of insufficient efficiency in the diagnosis of th e nature of pleural effusion,to explore the value of inflammatory markers LCR(lymphocyte-to-C-reactive protein ratio)、NLR(neutrophil-to-lymphocyte ratio)、LMR(lymphocyte-to-monocyte ratio)、PLR(platelet-to-lymphocyte ratio)in distinguishing malignant pleur al effusion from tuberculous pleural effusi on,in order to improve their early diagnosis and differentiation.Objective To compare and analyze the differences of neutrophil count,lymphocyte count,monocyte count,platelet,c-reactive protein and the levels of LCR,NLR,LMR and PLR between MPE and TPE,and to explore the value of LCR,PLR and LMR in the diagnosis of pleural effusion,in order to improve the differentiation efficiency between MPE and TPE.Methods 37 patients with MPE and 36 patients with TPE diagnosed in the second affiliated Hospital of Anhui Medical University from 2017 to 2021 were analyzed retrospectively.The levels of blood routine parameters,CRP and LCR,NLR,LMR and PLR were analyzed and compared between MPE group and TPE group.ROC curve was used to analyze the diagnostic value of LCR,LMR and PLR in predicting MPE.Results There was no significant difference in sex and age between MPE group and TPE group.Neutrophile count,lymphocyte count,platelet,LCR and LMR in MPE group were higher than those in TPE group,while CRP and PLR were lower than those in TPE group.There was no significant difference in monocyte count and NLR between the two groups.According to ROC curve analysis,the area under the curve(AUC value)of lymphoc yte count,CRP,LCR,LMR and PLR for predicting MPE was 0.701,0.772,0.796,0.773 and 0.771 respectively,the sensitivity was 0.865,0.649,0.676,0.892 and 0.676,and the specificity was0.528,0.806,0.861,0.639 and 0.778,respectively.Conclusion LCR and LMR in MPE group are higher than those in TPE group,while PLR is lower than that in TPE group.LCR and LMR,PLR have potential value in distinguishing MPE from TPE,which provides a new idea for clinical diagnosis of pleural effusion.
