| ObjectiveIn order to explore the positive effect of the family center maternity care(FCMC)model on the subjective well-being and breast-feeding quality,the FCMC was carried out,and compared with the routine obstetric nursing model in this study.To provide a theoretical basis for the formulation of practical clinical nursing measures.MethodThe 207 primipara were selected from Dalian maternity hospital between February and June 2017.The stratified multi-stage cluster random sampling method was used in this study,the experimental group consisted of 102 primipara sampled from family centered ward,the control group consisted of 105 primipara sampled from a general ward in the same hospital.When the primipara admitted to hospital,she was informed of the purpose and method of the study,and asked to fill out the general information questionnaire after informed consent of the parturient.Then the experimental group were implemented FCMC,and the control group were implemented routine obstetric nursing model.Total well-being scale and breast-feeding quality rating scale were filled by primipara 24 hours after natural delivery.After discharge(72 hours after natural delivery),nurses were followed up by telephone to fill in the breast-feeding quality rating scale of the primipara.The difference between the subjective well-being and breast-feeding quality of the control group and the experimental group was compared,and the feasibility of the family center maternity care intervention plan was discussed.SPSS 22.0 software was used for t test and χ2 test,and used to analyze the spearman rank correlation,etc.Results1.The total score of subjective well-being of primipara in the experimental group was significantly higher than that in the control group(P=0.000).The health concerns of primipara(P=0.003)and melancholy or cheerful mood(P=0.000)in the experimental group were significantly better than those in the control group.2.The dimensions of breast-feeding process before discharge(24 hours after natural delivery)(P=0.000)and the total score of breast-feeding quality evaluation(P=0.000)in the experimental group were higher than those in the control group.After discharge(72 hours after natural delivery),the primipara in the experimental group were superior to the control group in the dimensions of preparation before breast-feeding(P=0.022),in the process of breast-feeding(P=0.000),in the result of breast-feeding(P=0.000)and in the total score of breast-feeding quality assessment(P=0.003).The difference was statistically significant.3.After intervention,the primipara in the experimental group were better than the control group in terms of the episiotomy(P=0.000),the dystocia(P=0.030),the duration of the first stage of labor(P=0.046),the duration of the second stage of labor(P=0.001),the total duration of labor(P=0.043),the Apgar(P=0.045).The difference was statistically significant.4.There was a positive correlation between subjective well-being scores and multiple dimensions of breast-feeding quality before discharge(24 hours after natural delivery).There was a moderate positive correlation between energy and breast-feeding(r=0.282,P<0.01),and there was a significant positive correlation between energy and breast-feeding preparation(r=0.371,P<0.01),energy and the results of breast-feeding(r=0.320,P<0.01),energy and the total score of breast-feeding(r=0.372,P<0.01).Conclusion1.FCMC is helpful to improve the subjective well-being of primipara after birth.2.FCMC is beneficial to improve the quality of breast-feeding for 24h and 72h postpartum.3.FCMC is helpful to reduce the episiotomy rate and the dystocia rate of primipara,and shorten the labor process.4.There is a positive correlation between subjective well-being and breast-feeding quality of primipara,i.e.the stronger the happiness of primipara,the better the quality of breast-feeding. |
